Qualifications Licenses and Certifications: License and current registration to practice as an LPN in New York State required Valid driver's license may be required as determined by operational/regional needs. Education: High School Diploma or completion of LPN Program required Work Experience: Minimum of one year work experience as an LPN preferred Bilingual skills may be required as determined by operational needs. Home care/hospice experience preferred For CMHS only: Experience working in mental health and/or home care strongly preferred For Hospice only: Employee will be hired to perform nursing care duties in primarily one area of service (Home Care visits, Hospice Residence shifts, or Crisis Care visits). However the nurse will be cross–trained in each service area and will be expected to perform nursing care duties in all three Hospice services, as needed. Checks for allergies and MD orders before administering medications. Has working knowledge of Medicare guidelines, and performs in compliance with such guidelines. Follows appropriate plan of care after admission, including discharge and on-call as outlined by HomeCare policy. Consults RN and/or Clinical Supervisor regarding plan of care and care plan changes, including needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician). Supplements the nursing care needs of the patient as provided for by the RN in the plan of care including: providing services in accordance with program policies; preparing clinical and progress notes; assisting the RN or physician in performing specialized duties; assisting the RN in carrying out the plan of care; assisting the client in learning appropriate self-care techniques; and reporting changes in client's condition to the RN. This role is a 1099 contract opportunity, with care being provided at the patient’s residence. Patient Schedule Needs: Monday, Wednesday, Friday from 0900-1500 You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians. Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments, coordination of care. Perform various activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients. Provide education and training to the patient, their family, and/or the Home Health Aide. Work with patients to be done one-on-one in the patients home to provide for a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs. Minimum Qualifications: Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing Current State or Compact License as a Registered Nurse or Licensed Practical Nurse. Current CPR certification TB test and physical examination. Maintain required insurance. Good interpersonal skills. Hand-eye coordination and manual dexterity. Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients. Ability to operate under stressful conditions and make quick decisions. Must have dependable transportation and be willing to commute to the patient’s home.
