JOB DESCRIPTION: SUMMARY OF POSITION The LPN will provide mental health, substance abuse and outreach services to individuals who may have a mental health and/or substance use disorder. This role will work with community partners and local stakeholders to provide comprehensive care. As a part of the community-based or office-based team, the LPN will meet with individuals in the office and/or their own environment such as in the client’s home, hospital, schools/work, etc. to assess any medical/mental health needs, administer medication, and check vitals. A variety of clinical training opportunities are available based on program and interest. ESSENTIAL DUTIES & RESPONSIBILITIES Conduct nursing assessments for assigned clients. Arrange required lab work at specified laboratory and maintain record of results in client medical record. Provide clients and families assistance with medication pre-authorization as required and facilitate participation in sample programs and patient assistance programs as needed. Manage requests for refills. Attend team meetings as a member of the interdisciplinary team. Evaluate and document the efficacy, side effects, and complications of medications. Report findings to psychiatrist. Provide direct medical services and psychoeducation to clients and families on weekly basis in the community and/or office. Network with community physicians, inpatient facilities, and other community partner agencies for further assessment and treatment to ensure continuity of care. Connect patients with needed resources and referrals as appropriate. Provide client with medication and health and wellness education. As necessary, consult with Director of Nursing to maintain medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the psychiatrist. Maintain medical areas and records according to corporate, State of Florida, and JCAHO standards. Enters written documentation into Electronic Medical Record in a timely manner for all services rendered. Maintains departmental productivity expectations. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries. Competency with electronic medical records, computer systems, and documentation. Strong interpersonal communication skills in order to collaborate with team members and maintain professionalism. Accountable and self-directed with daily job duties. QUALIFICATIONS Education Level High school diploma or GED required. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Assumes responsibility for ordering medications · Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the resident's condition changes · Assists the attending physician with rounds and special procedures as needed · Transcribes, documents and implements physician's orders · Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering · Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance. Compiles a complete list of the patient’s medications to assist in reconciliation of medications across the continuum of care. Prioritizes activities for proper balance of patient care activities/assignments within assigned shift. Maintains and updates patient charts per unit policy.
