Job Requirements POSITION SUMMARY The Home Health Senior Liaison is responsible for driving business development, managing referral relationships, ensuring regulatory compliance, and identifying appropriate patients for Health First’s Home Health services. This role supports healthcare providers by assisting with clinical procedures, delivering direct patient care, and ensuring a smooth and efficient patient experience. The LPN plays a key role in maintaining clinical operations, supporting patient education, and contributing to a collaborative care environment. Responsibilities Key Responsibilities: Measure and record patient vital signs accurately Room patients and prepare them for exams and procedures Assist healthcare providers during examinations and procedures Provide patient support and comfort during visits Administer medications as directed Process patient phone calls in accordance with practice protocols Educate patients on healthcare plans, treatments, and follow-up care Communicate patient health status updates to providers Maintain and monitor medical supply inventory Stock and organize exam rooms Float to other practice locations as needed to support staffing needs Qualifications Minimum Education and Experience Requirements: Licensed Practical Nurse (LPN) eligible to practice in the State of Florida One (1) year of ambulatory care or hospital experience preferred Strong initiative with the ability to multitask effectively Excellent patient/customer service skills Clear verbal communication skills Strong teamwork and problem-solving abilities Licensure/Certification/Registration: Current LPN license eligible for practice in the State of Florida (Florida participates in the Nurse Licensure Compact) Basic Life Support (BLS/CPR) certification from the American Heart Association or Red Cross required within 90 days of hire
