Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met. Participates in management team meetings as indicated to discuss census changes, resident changes in status or care need, or complaints or concerns. May assist in evaluating work performance of nursing personnel and implements discipline according to operational policies and as directed. Informs appropriate departments of concerns or complaints and files grievances as necessary. Reports any incidents or unusual occurrences to the unit manager, assistant director or nursing or director of nursing and participates in the investigative process as needed. Participates in end of life care as required. Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs. Coordinates admission, discharge and transfer of residents. Obtains report from nurse he or she is relieving, provides report to nurse coming on duty and keeps the charge nurse or others (as appropriate) informed about the status of residents and related matters. Prepares, administer and documents medications following accepted practices. Consults with licensed or registered nurse, physician or licensed independent practitioner. Clarifies any order or treatment regimen believed to be inaccurate, ineffective or contraindicated by consulting with the appropriate licensed practitioner. Other duties as assigned. Required skills and qualifications: Current Licensed Practical Nursing (LPN) within the state of practice Current CPR and first aid certifications a plus One year of relevant nursing experience Flexible schedule, including availability to work evenings, weekends and holidays as needed
