Document all pertinent information in the resident wellness file. Maintain communications with resident’s family and or responsible party to ensure medical needs for the resident are being met. Contact resident’s attending physician when necessary and/or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to community team members as needed. Understand and follow infection control practices. Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks. Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions. JOB HIGHLIGHTS We are looking for an outstanding individual to join the nursing team as a Licensed Practical Nurse. You will support clinical services such as resident medication management, service planning and supervising the nursing staff. You will make a difference in the lives of residents by ensuring they receive excellent direct care. The scheduled hours are Monday - Friday, 8am-5pm, closed daily for lunch. Clinical/Front Desk position. Phlebotomy skills required. What You’ll Do Provides direct patient care and collaboration under the supervision of healthcare providers, and within scope of practice Performs and documents medical tasks to include medication administration wound care, observes reactions to treatments, and specimen collection such as urine, blood, and sputum based on provider orders Maintains medical equipment, ensures a clean and safe environment, and follows infection control procedures Maintains patient health records to ensure accurate and up-to-date records Performs other duties as assigned What You’ll Bring Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school Currently Licensed as a Practical or Vocational Nurse in state of practice required Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required At least three years of clinical experience as LPN/LVN in an ambulatory care, occupational health, or emergency department setting Work-life balance is at the foundation of how decisions are made and where Premise is headed.
