Key Responsibilities Perform initial and ongoing patient and family assessments and document findings according to unit and TriHealth standards. Develop and update individualized plans of care based on assessments, patient input, and established clinical pathways. Communicate assessment findings and care plans effectively with physicians and the healthcare team. Provide a safe, therapeutic care environment while upholding patient autonomy, dignity, and rights.
