DEFINITION: As the Clinical Supervisor, is responsible to oversee the clinical support staff to include work allocation, general performance, workflow efficiency, competency and accountability. Is responsible for staff training, including new employee orientation, for all aspects of health care delivery, including Joint Commission readiness. Performs as the primary liaison with the physicians to improve communication between the hospital inpatient unit, administrative and clinical staff, Office Supervisor and Administration. Attends meetings and participates in performance improvement efforts at both the clinic and department level. Develops and leads clinical staff in a team approach to ensure optimum flow of patient care daily for all providers. Ensures healthcare needs of patients are prioritized based on medical urgency. The Clinical Supervisor is able to perform various aspects of clinical duties in the clinic to include rooming patients when needed. Is responsible for setting up and overseeing Non-Stress Tests performed in the clinic. The Clinical Supervisor is accountable to the Physicians, Office Supervisor, and Administrative Director of the Department for the safe delivery of quality patient care. Patient population includes: all age groups to include adolescents, adults and geriatrics. Description The Home Health Nurse Manager is responsible for the management and oversight of clinical staff for the licensed home care agency and nursing staff supporting home infusion services under the guidance of the Director of Patient Services, S/he ensures that personnel issues are managed in compliance with Department of Health (DOH) regulations and contractual agreements. S/he works in collaboration with clinical leadership, administrative leadership, providers of care and referrers to ensure that the service maintains the highest standards of patient care and is responsive to the needs of the patient and family and the organization. Responsibilities Patient Experience 1. Supervises all clinical staff of Mount Sinai Home Health LLC home care agency and nursing staff supporting home infusion services. 2. Oversees the agency orientation for all newly hired clinical staff including introduction to agency policy and procedures, clinical skills training and assessment, documentation and EMR requirements and competency evaluations. 3. Ensures clinical staff adherence with written policies and procedures 4. Performs field supervision visits with all clinical staff on an annual basis at a minimum and remediation supervision visits as needed. 5. Monitors data regarding patient census staff productivity and collaborates with Director of Patient services to ensure financial stability. 6. Participates in the recruitment and selection of personnel. 7. Oversees the maintenance of patient clinical records and clinical staff personnel files. 8. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of patient experience. 9. Synthesizes quantitative and qualitative feedback regarding the patient experience and staff engagement to Nursing Management, staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 10. Follows up on all patient concerns and complaints in a timely manner, provides service recovery as needed and escalates patient concerns as appropriate. 11. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Quality and Safety 1. Supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. 2. Completes chart audits for quality and compliance monitoring 3. Demonstrates accountability for quality and safety practices and compliance and ensures maintenance of mandatory staff competency evaluations. 9. Fosters a professional practice environment through the facilitation of collegial interdisciplinary relationships, peer review, nursing research, innovation and joint practice arrangement through affiliating universities. Operations and Administration 1. Supports implementation of the mission, vision, goals and objectives of the agency, throughout the clinical service and individual patient care units. 2. Maintains patient/employee confidentiality in the management of information. 3. Monitors the availability and maintenance of equipment and supplies. 4. Evaluates clinical staff performance and mentors, coaches, counsels and disciplines staff as needed. 5. Conducts clinical staff meetings to ensure communication about progress, changes and compliance with all policies and procedures and regulatory agency standards. 6. Supports a shared decision-making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Responsibilities Include: Essential Duties and Responsibilities • Recruit, hire and train adequate and competent department staff. • Monitor and evaluate all aspects of the facility medication management. • Meet with all clients as needed to ensure best practice care, treatment and services are provided. • Communicate as needed with pertinent outside treatment professionals and in a manner consistent with MNA processes. • Ensure both admission and discharge processes are maintained to ensure continuity of care with respect to relevant nursing and medical issues. • Maintain the nursing schedule as per the facility and organization needs including but not limited to managing vacation requests, sick leave, new hire, and additional staffing requirements. • Ensuring facilitation of performance evaluations as required for all direct reports and implementing merit increases as approved by the Clinical Director. • Produce and maintain communication which provides detailed and useful information on all current clients. • Ensure quality nursing notes are entered for each patient every shift. • Organize and facilitate laboratory work per provider orders. • Ensure that an admission nursing assessment and care plan(s) for each client are complete per program policy. • Maintain confidentiality of protected health information. • Provide ongoing training as needed to existing nursing personnel. • Ensure effective communication between nursing staff and collaborating disciplines. Other Duties and Responsibilities • Ensure provider order for admission is present upon client admission to the program. • Organize and document required monthly nursing meeting and other necessary meetings as warranted. • Manage the maintenance of all nursing related spaces and equipment. • Ensure proper nursing documentation is entered for each client including but not limited to daily shift notes, weekly updates, treatment plans, treatment plan updates, contact notes and discharge summaries. • Review and document employee performance 90 days after hire, annually, and as needed per company policy. • Manage performance improvement/disciplinary action as warranted according to policy and document performance management as required. • Nurse Manager or designee to attend and present all necessary nursing related information in weekly staffing for all clients. • Coordinate with providers to ensure that necessary communication and documentation is maintained; specifically relating to the ongoing medical, psychiatric, nursing and medication management of client care. • Responsible for oversight of facility’s infection control/prevention program. • Responsible for ensuring that there are on-call nursing staff always designated. • Receive facility-related calls and come in for uncovered shifts when all other options have been exhausted (per-diem staff, part-time staff, full-time staff and Agency are unavailable). • Attend Nursing Leadership calls/meetings monthly, and as needed. • Assist with insurance issues as needed or designated by Vice President of Nursing Operations, or designee. • Coordinate and ensure that all admission requirements are completed per organization policy. • Ensure proper destruction / return of all medications is completed per policy and regulatory standards. • Maintain adequate inventory and supply of necessary nursing related items in accordance with monthly operational budget. • Assist with additional program needs as requested by the Vice President of Nursing Operations, Clinical Director or designee. • Seek opportunities to increase self-knowledge to improve the quality of client care provided. • Maintain compliance with Joint Commission and State/License regulatory standards and company policy. • In an emergency or declared state of emergency, may be required to stay onsite as a part of the emergency team until emergency has ended. • Demonstrates ability to identify and refer legal and/or ethical issues in client care to the Clinical Director or Director of Compliance. Qualifications: BSN from an accredited school of nursing, and current RN licensure Minimum of two (2) years experience Previous experience managing nursing operations, either in a residential or hospital eating disorder setting, is preferred Excellent communication, patient relations, and time management skills; safety awareness; and the ability to work as a member of a treatment team are all required #Rosewood
