Lurie Children's Hospital of Chicago Job Description General Summary: The Advanced Practice Provider (APP) works within the multidisciplinary clinical team to provide individualized patient care through history and physical assessment, ordering and interpreting diagnostic tests, medication ordering, prescribing and management, formulating diagnoses and patient/family education. They promote evidenced based practice by conducting or participating in research and academic activities. They provide support to colleagues and staff to promote the professional development of the advanced practice profession at Lurie Children’s and in the community-at-large. The role is unique to each division and in accordance with individual credentialing and privileging through the Medical Staff Office. Advance Practice Provider Transition into Practice Fellowship Ann and Robert H. Lurie Children’s Hospital of Chicago offers an Advance Practice Provider (APP) transition into practice fellowship for all new graduate APPs. All new hire APPs, even those with experience, are invited to participate in the fellowship. The APP transition into practice fellowship is designed to provide a supportive structure and strategies to facilitate the transition from the academic to clinical setting for newly hired APPs and institution to institution transition for those with experience. The first year of employment will consist of quarterly didactic sessions with integrated facilitator led peer supported transition sessions, structured mentorship, and precepted practice-based learning with standardized competencies. APPs in the fellowship are hired as paid employees of the institution. The fellowship is designed to aid the development of well prepared, clinically competent, APPs as well as to provide support, build confidence and skills to develop a collaborative practice. Essential Job Functions: Area Specific Job Accountabilities: Provides expert clinical care, education, consultation, advocacy research, leadership and mentorship. Manages primary, acute and chronic health problems through comprehensive history and physical examination assessment and diagnosis. This may include palliative and end of life care. Orders, performs and interprets diagnostic and therapeutic tests and procedures. Orders and prescribes pharmacologic and non-pharmacologic interventions and performs other therapeutic or corrective measures to treat illness and improve health status. Promotes the advancement of the profession at Lurie Children’s and the community-at-large. This may include publication, participation in professional organizations and providing educational support to the nursing staff. Collaborates and coordinates care with medical team to deliver evidenced-based care and establish best practices in clinical care. Participates in research design, implementation, and interpretation. Meets the Medical Staffs Allied Health Practitioners core measures of performance related to patient care, medical knowledge, practice-based learning and improvement, interpersonal communication skills, professionalism, and system-based practice. Knowledge, Skills and Abilities: Master’s degree or higher from an accredited school appropriate for certification as a Nurse Practitioner Physician Assistant. (CNS certification is not acceptable if hired after January 1, 2010) Two (2) years of relevant clinical experience is preferred. Licensure in the State of Illinois as an APRN/PA is required. Clinical privileges through credentialing by the Medical Staff Office achieved within 90 days of hire. Practices under the direction of advanced practice provider or physician providers until fully credentialed and privileged Basic Life Support certification required. Pediatric Advanced Life Support certification and/or other specialty certification is required in some areas within 6 months of employment. Any other certification required for specialty area. Description Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or Physician Assistant to join The Canning Thoracic Institute (CTI) as part of our Medical Intensive Care Unit (MICU) and Rapid Response Team (RRT). CTI operates collaboratively across the Northwestern Medicine health system to advance our mission of delivering the highest quality care for patients with diseases of the chest, promoting clinical innovation and discovery, and improving lung health in the Greater Chicago community. In this dual‑function role, the APP will work alongside leaders in pulmonary and critical care as part of an APP‑led team responsible for the management of medically complex ICU patients as well as the early recognition and stabilization of deteriorating patients throughout the hospital. The team cares for a wide variety of critically ill patients, including those with advanced lung failure, shock, sepsis, multiorgan dysfunction, and acute GI or oncologic emergencies. M edical ICU Responsibilities: Manage the care of critically ill patients requiring advanced monitoring and interventions. Participate in daily multidisciplinary rounds and collaborate closely with intensivists and the interprofessional team. Manage ventilators, vasoactive medications, and complex critical care therapies. Perform ICU procedures as credentialed (e.g., arterial lines, central venous catheters, bronchoscopy depending on skillset). Coordinate care transitions, including admissions, transfers, and discharges from the MICU. Apply evidence‑based practice guidelines to optimize clinical outcomes. Rapid Response Team Responsibilities: Respond to all Rapid Response and Code Blue activations throughout the hospital to assess and stabilize acutely deteriorating patients. Provide advanced clinical assessment, initiate urgent interventions, and determine need for ICU transfer. Collaborate with bedside nursing staff to identify early signs of deterioration and support escalation of care. Nurse Practitioner or Physician Assistant-Primary Care at Home, Full-time, Days

Description Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or a Physician Assistant to work in our Primary Care at Home department. The APP will travel to patients' homes in the western suburbs area of Chicago, or seeing patients in assisted living facilities. Schedule: Monday - Friday Call required after clinic hours, weekends and holidays Essential Functions : Demonstrates clinical expertise and expanded knowledge through patient care activities which may include diagnosis and management of a wide range of health problems. Responsible for health outcome of an assigned patient population. Confers with collaborating physician and other health professionals in area of clinical expertise. Develops appropriate educational systems for patients, families and staff in areas of expertise. Mentors graduate students in aspects of advanced practice and patient care. Actively participates in quality improvement and/or research pertinent to area of expertise. Responsibilities : Conducts comprehensive appraisals of patients' health status by taking health histories and performing physical examinations in clinical area. Provides direct clinical care to patients. Prescribes/orders medications, therapies; orders/interprets laboratory tests, x-rays and procedures according to clinical privileges. Evaluates patients to determine their need for further health care. Provides comprehensive teaching to patients and families. Documents in the medical record all care and education provided and/or procedures performed for which the Licensed Advanced Practice Provider is responsible. Refers to other disciplines as appropriate. Consultant to other disciplines in areas of expertise. Integrates research based knowledge and clinical expertise in decision making. Exemplifies a professional image in appearance, manner and presentation. Advocate for patients, families, coworkers, and the community. Maintains certifications and continuing education as required for licensure and privileging. Serves as a patient advocate and role model for 'Patients First'. Participates in unit/departmental/divisional or hospital committees/task forces as appropriate. Provides comprehensive teaching strategies for patients and their families. Develops and participates in programs to meet continuing education needs of staff. Serves as a role model and preceptor to students. Provides formal education to both internal and external groups pertinent to area of specialty. Participates in quality improvement and/or research initiatives for specific patient population as appropriate. Qualifications Required : Master’s degree from an accredited school/college of nursing OR master's degree from an accredited Physician Assistant Program Current license as a Registered Nurse in the State of Illinois. Current license as an Advanced Practice Registered Nurse in the State of Illinois. Certification as an Advanced Practice Registered Nurse by a national certifying body as approved by the Illinois Department of Regulation APRN Board. OR Master’s degree from an accredited Physician Assistant Program/Current license as a Physician Assistant in the State of Illinois.
