Consults with patient, support systems, and members of health care team to provide for acute and ongoing health care or referral of patient. Manages plan of care prescribed for patient. Initiates and maintains accurate records, appropriate legal documents, and other health and nursing care reports. Develops individualized teaching plans with patient based on overt and covert health needs. Counsels individuals, families, and groups about health and illness and promotion of health maintenance. Recognizes, develops, and implements professional and community educational programs related to health care. Participates in periodic and joint evaluation of services rendered. Conducts research and analyzes health needs of individuals and populations and designs programs which target at-risk groups and cultural and environmental factors which foster health and prevent illness. From obstetrics care to virtually any procedure within primary care to MAT for substance use disorders, there is opportunity at Family and Maternity medicine. However, APP spectrum of care could be tailored to training and comfort level. We serve patients across the lifespan and many of our family medicine physicians deliver babies at WBH. Because the majority of the group is comprised of teachers of family medicine, there is a true culture of learning and creativity which pushes us all to practice the most evidenced based medicine and continually improve. This APP would function as a primary care provider and carry a panel of patients in addition to acute care. Family and Maternity Medicine has one location attached to WBH. Dayshift and include Tuesday evenings. No overnight call and no weekend hours.
