Summary The Colorectal Surgery Section in the Michael E. DeBakey Department of Surgery is seeking a qualified Advanced Practice Provider to provide high-quality surgical care under the direction of a group of physicians at Baylor St. Luke’s Medical Center. Candidate will be expected to provide clinical excellence and scholarly enthusiasm. The provider will have direct patient care responsibilities. The selected candidate will be appointed as a faculty Instructor and be eligible for full faculty benefits. Job Duties Primary responsibilities include inpatient care of colorectal surgery patients in coordination with attending physician and resident physician team. This will include thorough pre-round chart review, engagement in collaborative rounds with attending physicians as well as independent rounding, documentation of clinical decision-making and treatment plans, coordination and execution of the day’s care plan, including wound management and other procedural interventions as needed, and frequent follow up on pt progress and testing to ensure patients are progressing through their plan of care. Obtain detailed history and perform physical examinations. See consults independently and/or with residents/fellows in various settings – floor, ICU, ED, etc. Formulate assessments and plans and evaluate the patient response to care Order and interpret appropriate diagnostic procedures, including, but not limited to x-rays, electrocardiograms, laboratory tests, and MR and CT imaging Ensure timely and accurate documentation in the EPIC electronic medical record system Prescribe appropriate pharmacological and non-pharmacological treatments (while inpatient and upon discharge) Educate patients on prescribed therapeutic regimens, home care and health maintenance Remove dressings and JP drains as indicated. Perform bedside simple wound debridement, suture closure, and bedside wound care Provide guidance and work direction to support staff, including, but not limited to nurses, social workers, and clinic coordinators Optimize discharge planning for all patients including participation in daily multidisciplinary rounds for floor and ICU patients, as well as timely and clear discharge communication to outpatient team Communicate with physicians, patients, and the family members of patients concerning patient care Provide orientation, direction and support to PA/NP students, medical students, and/or residents assigned to the service Promote a positive work environment with a team-based approach through supportive and respectful interactions with peers, staff, learners, and faculty Performs other duties as assigned; duties may change based on service needs. The ideal candidate will have prior Oncology experience. The schedule for this position is Monday - Friday, 8am-5pm. Key Functions Perform clinical patient care duties: Attend clinics with physician. Conduct preliminary assessments of physician's new and follow-up clinic patients. Dictate clinic progress notes on physician's follow-up patients, dictate PMEs on new patients, and dictate consultation notes on his new consults. Write admission notes and orders for physician's patients admitted into the hospital. Write outpatient orders, including those for chemotherapy, IV fluids, transfusions, growth factors, etc. Assist with physician's clinic-related paperwork. Assist with physician's miscellaneous clinic activities. Screen patients' telephone calls and refer to physician when appropriate. Evaluate physician's clinic and Emergency Center "drop-in" patients. Assume responsibility for physician's clinic patients (either scheduled or "drop-in"), even if staffed by another attending in physician's absence. Attend department meetings pertinent to patient care. Assist physician with in-patient care when needed. Work with another physician in physician's absence, if directed by physician. Perform other patient care related activities and various administrative duties pertaining to patient care and/or clinic operations as assigned by the Medical Director of clinic, the attending physician, or the MLP Manager. Perform a variety of research related duties during non-clinic hours, as directed by physician: Screen patients for eligibility for clinical trials. The services rendered will be delegated by the supervising radiation oncologist and in accordance with the policies established by the department of radiation oncology and Cancer Care Northwest existing policies. Responsibilities Responsibilities: Provides patient assessments through patient exams, medical history, interpretation of diagnostic procedures, and development and implementation of patient treatment plans. Provides consultation and education as a highly specialized practitioner within scope of practice. Managing follow-up patients after treatment completion. Functions independently as an advance practice registered nurse practitioner (NP) or physician assistant (PA) and consults with or refers patients to other health care providers or agencies as appropriate. Uses both advance nursing and medical knowledge in providing direct patient care. Provides diagnostic and pharmacologic management of the disease process and treatment side effects through ongoing assessment, intervention and education in collaboration with the physician. Consults on patients as requested by physicians and/or nurses and provides support as needed. Provides formal and informal educational opportunities for staff with emphasis specific to practice specialty. Integrates an individualized health maintenance program for clientele in the community and office Utilizes resources and problem solves when system barriers occur as the client repeatedly moves through acute, ambulatory, and home care settings. Provides knowledge in area of clinical expertise throughout the following phases of the illness trajectory: prevention, detection, diagnosis, acute care, terminal care/home care, rehabilitation, and survivorship to members of the multidisciplinary team involved in the assessment and management of individuals/families with cancer.
