Description We are looking for an Nurse Practitioner who speaks Spanish and English. The APN/NP is a specialty health care provider who provides medical and nursing services to individuals and families. The APN/NP is adept at diagnosing and managing acute and chronic illnesses, and emphasizes health promotion and disease prevention. Services include, but are not limited to, ordering, conducting and interpreting diagnostic and laboratory tests, prescribing medications, and teaching counseling individuals and families. Comprehensive health assessments with a broad scope of medical management for patients with acute and/or chronic health problems characterize the APN/NP’s practice. This position will work in Women's Health in the Specialty Clinic. Position available: Regstry,1PM-5PM, 1 day per week. ESSENTIAL DUTIES AND RESPONSIBILITIES Demonstrates clinical expertise in the diagnosis and management of a wide range of health problems. Develops medical treatment plans based on scientific evidence-based practice and evaluates outcomes. Effectively communicates and collaborate with physicians and other members of the health care team to resolve patient care problems and enhance care delivery. Mentors staff/students in aspects of advanced nursing practice and patient care Conducts comprehensive appraisals of patient health status by taking health histories and performing physical examinations. Initiate and participate in admission histories and physicals, medical consultations in related subspecialty, and discharge planning. Order diagnostic tests and procedures. Establish a diagnosis based on the history, physical examination and diagnostic test results. Formulates differential diagnosis. Evaluate results of diagnostic testing of laboratory tests, EKG’s imaging studies, and medical procedures. Make appropriate consultations and referrals. Develop a treatment plan based on diagnosis and evidence-based practice that is mutually acceptable by the patient and family. Determine the effectiveness of the treatment plan with documentation of patient care outcomes. Reassess and modify the treatment plan to achieve optimal health and medical goals for the patient. Teaches and counsels families in medical conditions, treatment plans, preventive care and anticipated outcomes. Documents in the medical record with SOAP charting all care provided or procedures performed by the APN/NP. Include differential diagnosis in the documentation. Education and Experience Graduate of a APN program Three-Five years’ experience in a clinical role. Experience in Staff education, and quality management preferred. Experience working with teams as a member and leader. Good customer relations and interpersonal skills. Preferred computer skills include, /Microsoft Word, Excel and Power Point. Req #610 Job Description Summary The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive health care services in compliance with the state Nurse Practice Act. Provides comprehensive care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating, and collaborating with patients, families, and other healthcare practitioners. The nurse practitioner visits patients and completes physical assessments as assigned in consultation with the primary care team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities. Essential Job Functions/Responsiblities: Patient Care Provides medical care in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Completes history & physical examinations, by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. Manages health conditions by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating health care services. Reviews clinical information, coordinates and oversees patient care in consultation with the attending physician and/or hospice physician. Assures that patient receives appropriate measures to control symptoms, through collaboration with the attending physician and/or hospice physician. Consults the Hospice Medical Director/Team Physician regarding the patient's plan of care. Perform face to face encounters within thirty (30) days of the third and subsequent hospice benefit certification periods and attest to the encounter. Communication Communicates and collaborates with other health care practitioners and the IDT team to create, review and revise the patient's plan of care. Consults with the patient's attending physician, primary care team and other healthcare practitioners regarding the patient's current condition, ongoing care needs and medical management. Communicates with other community health practitioners to coordinate the plan of care. Attends and participates in care management meetings as needed. Completes, maintains, and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. Educates/counsels’ patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the primary care team to meet the total needs of patients. Provides and maintains a safe environment for the patient. Assists the patient and family / caregiver and other team members in providing continuity of care.
