Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement). Completes assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope. Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge. Conducts post-discharge follow-up as indicated.
