Communicate with patients regarding test results and plan of care by phone or mail as directed by the provider. Job Description Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team. Under the supervision of the Director of Nursing, guided by the policies and procedures of the Ambulatory Care Center, and in collaboration with the physicians and other clinical staff in the Cardiology Clinic, the Clinical Nurse 2 is responsible for providing safe, efficient nursing care to patients in the UCSF Mission Bay outpatient Cardiology practice, supporting the Center for Prevention of Heart and Vascular Disease. Triage patients and responds appropriately according to protocol and nursing assessment. The Clinical Nurse 2 utilizes the nursing process (Assessment, Planning, Implementation and Evaluation) to provide safe and therapeutic clinical care. The Clinical Nurse 2 also provides clinical support to Providers and participates as part of the health care team to assure coordination and continuity of patient care, and interdisciplinary collaboration. The Clinical Nurse 2 participates in the development and implementation of educational programs and materials for patients and their family members.
