Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Licensed Practical Nurse is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The LPN will work on a Full-Time basis providing all services in the community and using a hybrid work model when not in the community. The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities. Overview The Licensed Practical Nurse will focus on providing high level patient-centered care in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. The LPN will also provide patient care by administering medications, processing patient phone calls per practice protocols, instructing patients regarding health care and treatments, and providing health care provider with information pertaining to patients' health status. May be required to float to other UFHP practices, as needed, for staffing. Qualifications Minimum Education & Experience Requirements Current Licensed Practical Nurse (LPN) eligible to practice in the State of Florida. One (1) year of ambulatory care or hospital experience preferred. Demonstrated ability to multitask and take initiative in a fast-paced clinical environment. Excellent patient/customer service skills. Strong verbal communication skills. Team-oriented with effective problem-solving abilities. Licensure, Certification & Registration Current Florida Licensed Practical Nurse (LPN) license or eligibility to practice in the State of Florida through the Florida Nurse Licensure Compact (NLC). Current Basic Life Support (BLS/CPR) certification through the American Heart Association (AHA) or American Red Cross, or ability to obtain certification within the first 90 days of employment.
