The Clinical Triage Specialist (CTS) (RN) - Access Center will compassionately deliver an exceptional patient experience and provide clinical support to CTS-MA team members by serving as a clinical resource. When appropriate, the caller’s symptoms will be assessed and triaged using approved nursing protocols and guidelines to assist in obtaining the appropriate level of care and/or self-care advice. JOB DUTIES AND RESPONSIBILITIES: Answers telephones, prioritizes clinical triage calls, follows clinical protocols, and coordinates services, as needed. Verifies patient demographic information and accurately enters the updated information into electronic health record. Serves as an escalation point for clinical patient issues and other POD team members requiring clinical support, and provides clinical advice based on clinical protocols and procedures. Manages and responds to escalated electronic patient messages whenever not answering inbound patient calls and uses clinical judgment to prioritize and accommodate patients. Creates a positive patient experience at every encounter, attempting to independently resolve any issues or concerns of the patient at the time of the phone call, within the scope of the role. Consistently meets productivity, schedule adherence, and quality standards as set by the Access Center. Utilizes all resources and guidelines at his/her disposal to effectively assess, prioritize, advise, schedule appointments, or refer calls when necessary to the appropriate medical facility or personnel. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Partners with other Access Center teams/PODs and respective practice clinical team on behalf of the patient to assist with clinical concerns, medication refills, or scheduling appointments. Other duties as assigned. EDUCATION: Graduate of an accredited nursing program. Active Registered Nurse licensure in the state of Pennsylvania and New Jersey or other nursing compact state and other states as deemed necessary by state law. General Summary The Office LPN assists the physician or mid-level provider by providing assessment’s, treatment, testing, and education for the patients of the practice. Must be well organized, high level of professionalism, excellent customer service skills, detail oriented, able to multitask efficiently, critical thinker and perform’ s in a fast-paced work environment. Essential Duties and Responsibilities Follows established clinic policy regarding patient safety measures such as infection prevention, safe patient handling and movement, critical lab notification, etc. to ensure that patient care is provided in an optimally safe environment. Administers medications (oral, IM, and SC) as ordered. Assists the provider with procedures, maintaining sterile field and managing instrument trays. Assists with examination as requested within scope of license. Phone triage. Acts as a general liaison between physician/APP and patient. Calls patients with normal/abnormal test results, answer’s questions and assisting physician with return calls. Documents chief complaint/visit reason and details according to provider preference. Escorts patients to exam room and obtains patient height, weight, and a full set of vital signs accurately. Initiates, reviews, and updates patient's medication and summary list according to provider preference. Performs point of care lab testing and obtains blood samples and other specimens for lab testing as ordered. Informs and sets up patients and/or guardian for My Chart at the time of service and informs and educates patients regarding the benefits of signing up for My Chart. Provides patient teaching and supportive services as needed. Provides treatments as ordered. Schedules diagnostic and therapeutic procedures, obtain’ s precertification and authorization as required by payers and ordered by provider. Trains LPN and MA in the clinic role. Meets commitments, works independently, accepts accountability, handles change, sets personal standards, stays focused under pressure, is a team player, and meets attendance/punctuality requirements. Education Graduate of an accredited LPN program and approved IV therapy course. Work Experience Nursing or other health care experience preferred but not required. Certification/License Current LPN license in the state of Missouri required. American Heart Association Basic Life Support required. Mental/Physical Requirements Considerable mental concentration required. Lifting up to 35 lbs., turning activities and nearly constant walking required. Standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching. Working Conditions Frequent exposure to infectious disease and hostile persons at times. Frequent exposure to communicable disease via blood and other body fluids. Minimal physical discomfort.
