Position Purpose The Nurse Navigator is accountable for coordination of care and educational support to patients. POSITION SUMMARY: The primarily role of this position is to proactively manage the outpatient population of Type 1 diabetes patients and growth-hormone requiring patients at Valley Medical Group Pediatric Endocrinology. This includes educating patients and their families with new-onset diabetes in an inpatient setting.
