Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness. Administers medications as prescribed safely; provides education on prescribed medications; recognizes signs and symptoms of adverse drug reactions. Demonstrates professionalism, accountability, and responsibility for decision making, quality nursing care, and knowledge base of psychiatric nursing care. Reviews, revises, and updates medical records within facility guidelines and timeframes. Nurse Practitioner - Complex Care Services Department Summary Advanced Practice Registered Nurse (APRN) to function in the inpatient Complex Care Service (CCS) at Boston Children’s Hospital. CCS is a long-standing and nationally regarded inpatient service in Pediatric Hospital Medicine within the Division of General Pediatrics that serves hospitalized children with medical complexity, who have complex chronic conditions and functional limitations. Common reasons for admission to CCS include infections (aspiration pneumonia, pyelonephritis), chronic condition exacerbations (epilepsy, digestive dysmotility), and post-operative recovery following surgery (baclofen pump placement, gastrostomy, spinal fusion, etc.). This role will provide patient evaluation, assessment, and facilitate care for the patients on the CCS service and will implement interventions tailored to improve systems for patient care and enhance patient safety for this population. Drug Administration Functions Prepare and administer medications as ordered by the physician. Verify the identity of the resident before administering the medication/treatment. Ensure that prescribed medication for one resident is not administered to another. Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor. Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies. Ensure that narcotic records are accurate for your shift. What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey. Perform initial and ongoing assessments to monitor patient condition and adjust care as needed. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners.
