Primary Responsibilities: Weekend coverage: Performs skilled nursing activities such as: Completes assessment of patient for effective symptom management. Incorporates nursing skills, communication skills, and emotional intelligence skills to effectively assess problems/needs. Perform treatments and interventions in accordance with the plan of care and physician’s orders. Provide information and education regarding disease progression and anticipated needs at end of life. Provide instruction to the patient/care giver in accordance with the plan of care or changes in plan of care. Collaborate with patient, family/caregiver, and practitioner regarding goals of care and changes in treatment plan. Follows precautions based on plan of care. Provides appropriate emotional support during all conversations with patients/caregivers. Conducts follow up and collaboration calls as needs arise or as requested by the team. Communicates and coordinates as follows: During shift answers phone at time of call or returns call to Triage within 5 minutes (unless actively engaged in patient care). Provides real-time updates to supervisor regarding visit arrival and departure times per protocol.
