The Home Hospital Registered Nurse is responsible for providing the essential elements of the inpatient care delivery model into the patient’s defined living space using leveraged technology and remote clinical care coordination. The Home Hospital RN provides quality care to patients through the implementation of the nursing process applied through a team nursing model. The Home Hospital RN will access and document the care of patients and will work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient’s home while maintaining high quality and safety standards.
