In an environment of continuous quality improvement, t he Registered Nurse is responsible for rendering professional nursing care and directs and guides patient teaching and activities of ancillary personnel . Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times. Job Functions: Participates in bedside shift report/handoff. Performs hourly rounding. Obtains required patient assessments and reassessments per nursing policy. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises as needed per hospital policy. Coordinates timely, adequate teaching to patient/family/significant other in preparation for discharge and documents appropriately. Implements and monitors nursing interventions. Consistently documents assessments, interventions, and outcomes in Meditech within required timeframe. Participates in interdisciplinary rounding and collaborates with team members to identify patient needs. Recognizes changes in patient conditions and communicates with physician using SBAR. Effectively manages patient care and delegates appropriately according to the needs of the patient and the skill of the caregiver. Utilizes appropriate interpersonal communication which facilitates productive relationships with other team members. Facilitates a positive and caring relationship with others by utilizing trust and mutual respect. Establishes a therapeutic relationship between patient, family and self, based on empathy and trust. Promotes a positive patient experience. Seeks help from appropriate resources as needed. Maintains current knowledge of and practices within the scope of hospital policies. Clarifies any unclear communication between members of the healthcare team. Uses proper channels of authority in communicating information. Collects Data related to physical, psychological and psychosocial health status. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises same as diagnosis and needs dictate. As part of the nursing process, performs admission, handoff, transfer and discharge procedures. Coordinates with physician’s plan of care, the admission assessment and discharge needs. Maintains compliance in RQI. Assumes all other duties and responsibilities as necessary. Charge Nurse: In addition to above essential functions, the charge nurse will also : Complete daily nurse leader rounds. Perform shift checks as assigned. Assists with completion of chart audits for nursing documentation as required. Complete staff assignments taking into account the skill level and acuity of patient assignment. Manages throughput on the nursing unit and make changes to assignments as needed. Reports to the nursing unit manager any problems in regards to performance, safety, staffing, behavior, etc. Assists with conflict resolution in the absence of the nursing manager in collaboration with the house supervisor. Role models positive behaviors with staff including integrity, fairness and consistency. Agent for change for the department. Assigned and assist as super user for completion of incident report. Minimum Education/Experience Required: Current licensure in the State of Ohio or if working in WV will need a WV license as a Registered Nurse. BLS required upon hire. ACLS within 1 year of employment required. Special Knowledge, Skills, Training: Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity, proper lifting techniques and body mechanics. Summary Occasionally JOB SUMMARY/PURPOSE The Registered Nurse (RN) is accountable for utilizing the nursing process to provide safe, age-appropriate, direct and indirect Hospice patient care, while practicing within the scope of practice of the Oregon State Board of Nursing. DEPARTMENT DESCRIPTION The Hospice teams at Samaritan Health Services support patients and family members through the final stages of their illness. The Samaritan Evergreen Hospice House in Albany is a 12 bed inpatient facility that provides symptom management, respite care and room and board. Services offered include skilled nursing care, pain and other symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, bereavement support and medication and equipment as appropriate. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon RN Licensure required. Healthcare Provider Level BLS required. BSN and/or previous HH/Hospice or case management experience preferred. Previous broad-based nursing experience to ensure specialized skills needed for HH/Hospice Nursing preferred. AGH Hospice Inpatient: Successful completion of Food Handler's course required within 30 days of hire. Must be able to pass Criminal Records Check upon hire and every three (3) years thereafter. KNOWLEDGE/SKILLS/ABILITIES Adaptability/Flexibility: Ability to respond quickly and appropriately to urgent medical situations that may occur with high risk or diverse patient populations. Ability to recognize and report changes in patient status. Medical Terminology: Knowledge of medical records, procedures and terminology. Ability to read, interpret, and apply policies, procedures, laws and regulations. Patient Care: Possess the skills and knowledge needed to assess and treat human injuries, diseases and or behavioral conditions in collaboration with MD orders. Ability to recognize symptoms, understand drug properties and interactions, and employ treatments and preventive healthcare measures. What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey. Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home. Perform initial and ongoing assessments to monitor patient condition and adjust care as needed. Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans. Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance.
