Registered Nurse (RN), Home Health Care - Full-Time, Days

Job Description Join our UChicago Medicine Home Care team as a Registered Nurse The Home Care RN i s responsible for providing patient care on an as needed basis. The nurse plans, organizes and directs home health services and is experienced in nursing, with emphasis on community health education. Eligible for a Sign-On Bonus up to $15,000 UChicago Medicine fosters a concern for patients that extends well beyond the walls of our facilities. We have expanded our home care services to keep pace with shorter hospital stays, evolving technology, changing insurance reimbursement and a growing elderly population. Essential Job Functions Assesses patient and family to determine ongoing home care needs and progress toward goals. Provides a complete physical assessment and history of current and previous illness(es) Initiates plan of care as assigned, including preventive and rehabilitative nursing procedures. Monitors patient response to interventions. Develops a care plan which establishes goals, and incorporates therapeutic, preventive, and rehabilitative nursing actions, as well psychosocial support as needed. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Prepares clinical notes and updates the case manager after each visit as appropriate. Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with clinical supervisor and/or weekend coordinator as needed to assure continuity of care. Responsible to come into agency as needed to receive assignment, supplies. Communicates with clinical supervisor and/or weekend coordinator regarding new referrals and hospital discharges. Accountable to have all on-call equipment and supplies to appropriately carry out responsibilities. Understands role of Supervisor on Call / Administrator on Call and Weekend Coordinator to utilize for resource and emergency back up after hours. Completes all required documentation, adheres to electronic and paper documentation submission time frame. Represent the agency as a 24-hour service, respond to phone calls within agency time-frames, and make visits in accordance with agency policy. RNs supervises LPNs and Home Health Aides. Commits to 100% patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others, including patients, families, visitors, physicians, students and co-workers. Required Qualifications Current state of Illinois nursing license RN as applicable. Current CPR is required by AHA. One to two years of recent acute care experience in an institutional setting required. One year of home health care experience preferred. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits. Clinical Nurse Educator, NICU, Registry

Job Description Join UChicago Medicine, as a Registry Clinical Nurse Educator (RN) in our NICU Department . In this role, the Clinical Nurse Educator is a nurse with previous educational experience whose focus is on supporting existing staff development and nursing education. This nurse is an educator, consultant, role model, staff/patient advocate, change agent, leader and cost effective professional. The Clinical Nurse Educator elevates the standard of professional nursing practice at UCM by developing, coaching and mentoring staff to become expert clinicians and leaders. The Margaret M. and George A. Stephen Neonatal Intensive Care Unit (NICU) at Comer Children’s Hospital provides Level IV NICU care, which is the highest level of newborn care according to the American Academy of Pediatrics’ standards. Comer Children’s is one of only a few children’s hospitals in Illinois that provides Level IV NICU care just steps away from our birth center. Our pediatric surgical subspecialists on staff can perform advanced surgical procedures when needed. We also offer extracorporeal membrane oxygenation (ECMO), a type of life support for infants with heart or lung problems. Essential Job Functions: Utilizes standardized and customized assessment tools and testing methodologies to measure staff knowledge and skill in core nursing practices and within specialty areas. Collects data regarding employee learning needs, at least annually, through the methods of observation, interviewing, focus groups and instrumentation. Interprets the medical center’s strategic imperatives, policies, guidelines, and other internal demands in order to design education that supports these strategic goals and key objectives. Offers recommendations for the deployment of educational resources to enable the achievement of organizational goals. Interprets health care trends and incorporates current research into the design of learning needs assessments and the identification of learning needs. Collaborates with key staff to develop a comprehensive annual educational calendar. Participates in the design of centralized and unit-based preceptor education and development initiatives. Prepares educational objectives, lesson plans, handouts, self-learning modules, instructional manuals, and tests for programs. Serves as faculty in organization wide education events, as appropriate. Evaluates all learning activities using objective performance criteria and recommends corresponding changes in education plans. Utilizes appropriate tools and testing methodology for assessing changes in learners’ knowledge, skills, and clinical practice as well as for determining education project/program effectiveness. Develops knowledge and skill based measures to collect performance data and integrates measurement activities and findings into quality projects. Researches available funding sources and submits funding proposals in order to implement support and expand new or existing programs to meet the educational needs. Assure the timely completion of each section of the MDS by proper department, signing of the form by staff on EMR, and its resident appropriateness. Confer with each department regarding any problems that occur in its sections of the MDS and arrange for re-education when necessary Review diagnosis coding and sequencing with physician quarterly. Update/ clarify ICD-9 coding as appropriate. Conduct weekly MDS Pre-planning and Medicare meetings to ensure timely and accurate completion. Monitor and assist staff in all departments regarding EMR charting, documentation and achieving Care Plan goals in accordance with the resident’s MDS. Communicate resident care plan and progress to staff, resident and family. Conduct in-service training of nursing staff every three months as needed on care plans and documentation. Audit sample EMR charting monthly to ascertain proper nursing charting and documentation of care given and directly confer with those employees who are not properly documenting. Record, report and follow up discrepancies in documentation which are not consistent and/or do not meet program standards. Work closely with the Director of Nursing and other department heads to ensure consistency in care. Coordinate resident Care Plan with physical, occupational, respiratory, and speech therapy department to ensure proper care and treatment when clinically appropriate. Follow up on nutritional assessments by the dietary consultants with reporting to the nursing staff and appropriate documentation. Maintain the confidentiality of resident information and honor his/her personal and property rights. Comer Emergency Department is a 21-bed, Level 1 Pediatric Academic Trauma Center dedicated to providing high-quality, evidence-based care to the children of South Side of Chicago and beyond. With 2 trauma bays, 1 special procedures room and a dedicated resuscitation bay, we are equipped to handle the most critical situations. The Adult Emergency Department consists of 55 patient care rooms including a rapid assessment unit, six trauma/resuscitation suites and separate rooms for infection isolation, care of immune compromised patients, and psychiatric consultations. In the general care area, 45 rooms are fully equipped with critical care monitoring capabilities. This position is a 100% onsite opportunity . You will need to be based in the greater Chicagoland area. A staff nurse is a registered professional nurse (RN) who manages patient centered care that is outcome directed. Patient centered care principles and professional nursing practice standards provide the foundation for the nurse to assess, plan, implement and evaluate the care provided. The staff nurse assumes the primary responsibility and accountability for all patient care personally provided and that provided by those directly under her/his supervision.
