Description Kindred Hospital New Jersey - Rahway is a 34-bed long-term acute care hospital located on the 4th floor within the RWJ University Hospital based in the medical district of Rahway, close to several educational institutions and Madison Avenue Park. Job Summary Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharge planning process. Revises the plan of care as indicated by the patient’s response to treatment and evaluates overall plan daily for effectiveness. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age. Receives physician’s orders, ensures transcription is accurate and documents completion. Administers medication utilizing the five rights of medication administration reducing the potential for medication errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate. Assists physicians with examinations, treatments and special procedures and performs services requiring technical and manual skills within scope of practice. Performs treatments and provides services to level of licensure. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians, and other healthcare providers. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Interprets data about the patient’s status to identify each patient’s age specific needs and provide care needed by the patient group. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Initiates or assists with emergency measures for sudden adverse developments in patients’ condition. Answers telephone, paging system, patients’ call lights, anticipates patients’ needs, and makes rounds of assigned patients and responds as appropriate. Qualifications Required: ASN or Nursing Diploma from an accredited school of nursing Strong communication and organizational skills Proficient computer skills Preferred Med./Surg. Telemetry and/or ICU nursing experience CCRN Certifications And Licenses Required BLS upon hire and ACLS Certification within 6 months of start date Active New Jersey Registered Nurse License Documented completion of TCAR – Trauma Care After Rescusitation every 2 years Completion of Critical Care Course after 3 months Scheduling Requirements Day shift, 35.3 hours, 7a-7p Monday – Friday and Every Other Weekend Essential Functions The Registered Nurse provides nursing care in the health care system within the scope of practice defined by the Nurse Practice Act and regulatory standards and assures awareness of ethical standards of practice as defined by the ANA code of ethics. The Registered Nurse assures safety in practice and is inclusive of the principles of evidence-based practice by maintaining a realization of own individual level of knowledge and experience and assumes responsibility for advancing knowledge of self and others of the patient care team by communication with and utilizing resource persons and programs available. The Registered Nurse is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors. The Registered Nurse is responsible for knowledge of the patient population served. The Registered Nurse is responsible for supervising and directing the delegation of tasks to appropriate team members and collaborates with other patient care providers. The Registered Nurse is responsible for consistently setting and evaluating priorities through appropriate interventions with measurable outcomes toward improvement. The Registered Nurse is an individual practitioner and the interdisciplinary team manager, utilizes the nursing process to integrate all aspects of the provision of care and individualizes the plan of care to address a specific patient’s needs.
