Complete MAR/TAR audit each month. Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse). Act as liaison for pharmacy services to ensure effective services for residents. Monitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications. Complete Medication Observations and skills checks for medication care managers at the direction of the RCD. The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. This unit is responsible for the processing of Medicaid eligibility for consumers who receive services through one of the HCSP programs. Home Care Services Program (HCSP) is recruiting two (2) Staff Nurses, to function as Staff Nurses in HCSP/ CASA 11 – Care at Home/ Home Bound Unit, who will: - Conduct in-home or virtual assessments at the request of HRA using a State approved assessment instrument to evaluate the client’s ability to safely perform activities of daily living and the degree to which the client requires assistance for each function and task. The completed assessment would be entered into the New York State Department of Health (NYSDOH) and/or HRA computer system. - Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment. The development of a plan of care that takes into consideration client’s home environment and social support system. - Complete in-home and virtual visits and submit the completed HRA/NYSDOH approved assessments and recommendations forms no later than five (5) workdays after the date of assignment. - Conduct case reviews of new applicants, service change and re-authorization cases, and participate in special projects. The reviews consist of screening all documents related to the request for service and may include contacting the client’s physician or other resources for additional medical documentation and information as necessary. - Develop a plan of care for the type and amount of home care services to be provided to the client after reviewing all required case documentation. - Prepare documentation for and testify at Fair Hearings and at other legal proceedings to explain clinical findings and determinations regarding the plan of care. - Provide consultation to HRA Medical Review Team and social service staff at HRA’s request. - Complete self-audits of State approved assessments and medical review of charts. - Work toward obtaining PRI/screen certification and conduct assessments of clients transitioning to a Nursing Home Facility. Bargaining Unit PEF Job Summary The Department of Nursing at SUNY Downstate Health Sciences University is seeking a full-time Registered Nurse 1, NS24 SICU. The Registered Nurse 1 (T&RC) is a full performance level nurse position and possesses a license as a registered professional nurse in New York State; under the general direction of a higher level registered nurse or licensed health care provider, independently provides direct nursing care to individuals in accordance with Article 139 of the Education Law; supervises nursing care activities; and provides clinical supervision and training to Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff. Illustrative Duties Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Participates in the development, implementation, and revision of individual care plans as a member of a multidisciplinary treatment team. Modifies nursing care plans based on individual condition and responses to care and treatment. Triages individuals by inquiring about illnesses, injuries, and symptoms, and records findings and observations. Assesses findings, and either refers individuals directly to a physician in emergency cases or initiates appropriate remedial action; as directed, assists physicians with treatment functions. Provides education and counseling to individuals and families regarding health, medications, procedures, and treatment goals. Practices and ensures compliance with established infection control policies and procedures. May train paraprofessional staff by demonstrating nursing techniques and providing instructions on use of techniques and nursing equipment.
