In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. In this role, the Nurse Associate (RN) is responsible for coordinating diagnostic and therapeutic regimens for a specified group of patients, and maintains cohesiveness between clinical research, physicians, and patients. The Nurse Associate coordinates physician activities with referring physicians, nursing staff, hospital administration, ancillary services, support services, insurance companies and other departments/sections and agencies as needed, and assists the physician with a wide range of activities necessary to manage clinical research programs. The Case Manager collaborates with the provider to coordinate inter-disciplinary services which may include nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services. The Case Manager is accountable for attaining the best possible patient outcomes including identification and focused oversight of patients at high risk for hospitalization. The Case Manager provides evidence-based, patient-centered care at the patient’s place of residence, using critical thinking and decision-making skills. Qualifications REQUIRED QUALIFICATIONS: Education: Completion of nursing program Experience: Experience as RN in a healthcare environment for two years or more. Valid State Driver's License with proof of active auto insurance. Other Skills/ Knowledge: Excellent interpersonal and communication skills.
