Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in the Med/Surg/Telemetry Unit. Rockland Psychiatric Center is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. Candidates from outside or within State Service can apply. As a Registered Nurse 2 (Community), you would directly provide, oversee, and coordinate nursing and health related care services for a caseload of individuals in a community setting, such as a Family Care Program, Mobile Integration Team, and outpatient clinic; perform clinical case management activities; serve as a member of an interdisciplinary treatment team; and provide nursing-specific clinical, consultative, and instructional assistance to individuals and their families, other disciplines, and health care providers.
