Communicates with billing to verify atypical insurances as requested by referral sources and to support UR and Clinical Staff. 8. Identifies pre-admit evaluated patients and works to determine home care eligibility and appropriateness for home care . Communicates with department manager to determine acceptance of referral. 9. Follows referral department guidelines regarding orders required to initiate home health services . All physician orders/approvals are noted in the referral orders. 10. Prioritizes completion of referrals based on clinical priority, patient safety, and insurance precertification. 11. Effectively manage the time it takes to complete referrals by understanding what information is desirable vs necessary. 12. Uses critical thinking and problem - solving skills to resolve referral issues limiting the need to involve the referral source. 13. Utilizes clinical judgment to determine optimal services to manage a patient safely in the home setting. 14. Adheres to referral order documentation guidelines developed to guide agency initiatives. 15. Maintains productivity standards as defined by the department standards. 16. Obtains prior authorizations and pre-certifications as required by insurer . 17. Completes e-tracking and Forcura case final verification to inform clinical departments on admission state, diagnoses and priority of ordered care. 18. Keep the Intake clinical manager informed of any problems communicated by referral sources. Report any changes or trends noted in referral patterns to Department manager. Keep abreast of third-party regulations and insurance companies and HMO’s. Identifies opportunities to generate potential referrals for all agency services to meet the care needs of the patient. 19. Demonstrates required knowledge of Home Health Foundation services for which a patient may be eligible. 20. Primary Duties And Responsibilities Deliver direct oncology nursing care, including comprehensive assessment, individualized care planning, symptom management, and evaluation of patient outcomes for adult oncology patients and families. Administer antineoplastic agents and supportive therapies per protocols and ONS standards; verify orders, complete independent double checks, and monitor for and manage infusion reactions and adverse events. Manage central venous access devices (e.g., ports, PICCs); perform access, maintenance, blood draws, and blood product transfusions per policy; ensure safe use of infusion equipment and PPE. Provide patient and family education on diagnosis, treatment plans, self-care, side-effect management, and discharge instructions; coordinate multidisciplinary resources and community referrals. Document care in Epic in accordance with regulatory and institutional standards; maintain accurate medication administration records, flowsheets, and patient education documentation. Participate in quality and safety activities (e.g., safety huddles, infection prevention, hazardous drug handling, Joint Commission readiness) and support clinical research protocols as applicable (e.g., protocol adherence, specimen handling, adverse event reporting). Collaborate with physicians, advanced practice providers, pharmacists, social workers, and research staff; contribute to effective team communication, handoffs, and patient throughput. May serve as charge nurse, preceptor, or mentor; provides functional direction to licensed and unlicensed support personnel in accordance with scope of practice and institutional policies. Knowledge, Skills And Abilities Proficiency in chemotherapy/immunotherapy administration per ONS guidelines; working knowledge of oncology pharmacology and side-effect profiles. Competence in central line care and infusion therapy; ability to administer and monitor blood products per policy. Ability to recognize and respond to oncologic emergencies (e.g., infusion reactions, febrile neutropenia, tumor lysis syndrome) and escalate care appropriately. Strong patient education skills with ability to tailor communication to diverse populations and varying health literacy levels; commitment to culturally competent, trauma-informed care. Proficiency with EMR systems (Epic preferred); accurate and timely clinical documentation and electronic medication administration. Knowledge of infection prevention, hazardous drug handling (USP ), PPE requirements, and universal precautions. Day shift charge nurse completes warm hand off to evening charge nurse of any issues or concerns that had arisen throughout the day. Evening charge nurse to notify the lab and radiology when the last patient has left the clinic. Evening charge nurse to communicate any clinic exam rooms that require a deep clean to the building operations manager and/or the cleaning company email as appropriate when the last patient has left the clinic. Completes the fridge/freezer temp check/logs. Review required supplies and notify facilities of needs for the department clinic. Complete daily code cart check and weekly POC testing check. Other duties as assigned.
