Identifies and reports all risk management issues in a timely manner. Responsible for phone management of patients. Triages phone calls utilizing critical thinking and assessment skills to evaluate the chief complaint and urgency of the problem. Triages phone calls to determine acuity to assist in scheduling appointments. Provides comprehensive medical record documentation. Documents all communications and maintains patient confidentiality. Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues.
