Medication Administration a. Demonstrates knowledge of and follows SSH policies and procedures for administering, transcribing, and recording medications. b. Completes medication reconciliation process following SSH policy and procedure. c. Demonstrates proper procedure for the documentation of narcotic withdrawal, administration, verifies count, wastes per policy and resolves narcotic discrepancy. 2. Plan of care/Documentation/Patient Family Centered Care/Patient Experience Develops, discusses, and communicates a prioritized problem list/plan of care for each patient. a. Develop, evaluate and update individualized plan of care for patient and documents outcomes. b. Initiating admission assessment within 8 hours of admissions, identifies and documents patient/family/significant other teaching needs upon admission and throughout hospitalization. c. Continues assessment/reassessment and identifies care needs within established nursing practice. d. Documents all patient care following the department of nursing policy, unit based standards, disease processes (CHF, PNA vaccines) and nurse sensitive indicators (falls, skin, CAUTI) e. Assess/reassesses and documents patient's level of comfort utilizing the appropriate pain scale and the patient's response to each intervention both pharmacological and non-pharmacological per South Shore Hospital policy. f. Works on behalf of patient /family. Seeks help to represent patient/family when they are unable to represent themselves. g. Raises ethical questions and concerns with clinical team. Seeks available resources to help formulate and understand ethical decisions. h. Actively communicates plan of day via white board updating every shift. i. Facilitates discharge/transfer by goal of 12:00pm, communicates barriers utilizing chain of command. 3. Safety/Quality - Foster's a "Culture of Safety" through personal ownership and commitment to a safe environment. a. Verifies patient identification with two identifiers prior to the start of any invasive procedure, including "time out", administration of care, medications, labeled specimen's an documents confirming the correct patient, procedure, site, equipment and consent. b. Complies with the current CDC hand hygiene guidelines through proper handwashing. Adheres to universal precautions, makes appropriate use of personal protective equipment at all times and appropriately disposes of hazardous materials. Maintains awareness of MSDS sheets and how to access. c. Ensures environment meets regulatory requirements at all times. d. Understands and is able to demonstrate individual roles and responsibilities in the event of hospital codes/emergency preparedness. 4. Professional Development: Assumes overall responsibility for own professional development by incorporating evidenced-based practice, research, and performance improvement initiatives as a part of ongoing nursing practice. a. Obtains at least 5 contact hours per year in area of practice. b. Practices within the legal boundaries of MA Nurse Practice Act. Directs other licenses and non-licensed personnel as assigned. c. Alongside medical providers, case managers, social workers, behavioral health team members and other HIV nurses, you will provide high-quality nursing care to patients consistent with the philosophy of patient centeredness, non-judgement, and team-based care. You will serve in a key clinical role to the care team with primary responsibilities of HIV and other chronic disease management, care coordination, advice/triage needs for the team’s patients, and ensures excellent communication of the care plan across team members. In this role, you will conduct clinical nursing assessments and triage for team patients during routine and urgent medical visits in the outpatient clinic, as well as during outreach visits to shelters, drop-in centers, street venues and patients’ homes. You will also provide care coordination, medication adherence support, health education, and behavioral risk reduction counseling for HIV primary care patients. This role offers a unique opportunity to promote linkage, and retention to HIV primary care, with the aim of supporting people experiencing homelessness and living with HIV to achieve and sustain health and wellness. This position is funded until the end of March 2028 with the possibility for renewal. If this funding is not renewed, this position will end March 31, 2028. Responsibilities: Provides outpatient primary care nursing including medical assessments and triage services for HIV team patients coming in for same-day or scheduled visits; conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient’s health concerns using approved protocols and resources to advise appropriately. Documents patient information accurately and in a timely manner in the EHR. Implements the patient-centered plan of care, evaluates outcomes, and regularly communicates the plan of care, patient status, and progress toward goals with the patient and care team members; conducts comprehensive intake assessments for new HIV team patients. Administers vaccines, injectable medications, and provides wound assessment and wound care as indicated; provides medication adherence assessments and counseling. Provides directly observed therapy when indicated with support of outreach team members. Coordinates care with integrated HIV team behavioral health staff. Along with other team members, visits patients at shelters and other outreach sites to provide nursing care and care coordination services and identify barriers to medication adherence and engagement in care. Provides chronic disease management support for HIV as well as other common co-morbid medical conditions such as diabetes and hypertension. Identifies any structural barriers patients face in accessing medical care and adhering to HIV medications, and works with the interdisciplinary team to address those barriers (e.g., housing, transportation, drug treatment, food, clothing, benefits, etc.) Assists medical providers in coordinating follow up related to the provision of medication (such as buprenorphine and naltrexone) for opioid use disorder and other substance use disorders. Coordinates preventative services for HIV team patients including, but not limited to, education, preventative medications, vaccinations, testing, and referrals. Promotes a compassionate and therapeutic environment that is responsive to the unique patient population and extends to members of the care team; demonstrates initiative and commitment to continuously improve services and processes that positively impact patient care and organizational goals. Participates in training and orientation of new nursing staff as directed by Supervisors. Qualifications: Licensed as a Registered Nurse in the Commonwealth of Massachusetts. Certification as AIDS Certified Registered Nurse (ACRN) preferred; however, this can be obtained after being hired, and program funding available to support this training . The Intake RN must answer calls entering the queue (or coming in on direct extensions) and process demographic, clinical and insurance information in a manner consistent with service excellence and that ensures efficient and effective call processing, leading to a patient admission to Pembroke Hospitals program or providing appropriate information to the caller. The Intake RN is an adjunct to the department and is available as an immediate resource to non- clinical staff to assist them in ensuring appropriate medical screening in order to facilitate timely and expedited placement of patients within Pembroke Hospital facilities who meet admission guidelines. Responsibilities Complete intake call sheet on all referrals including for inpatient as appropriate for the position. Gather all necessary demographic, clinical and financial information on call sheet. Ensure information is accurate and complete as call sheet is transmitted to hospital clinical and business office staff. Requests evaluation from ESP/ED/Referrer as appropriate. Assist Intake Coordinators when requested in interpretation of lab values, and diagnostic testing within Pembroke guidelines in order to facilitate appropriate referrals to facilities. Reviews for first call acceptance of appropriate patients based on Pembroke protocols. Notify the units of pending admissions. Send completed call sheets and insurance verification/authorization to Hospital Unit, Utilization Review and Business office. Maintain accurate, up-to-date bed availability including update hospital staff of pending admissions or cancellations. Update external bedboard on regular basis. Adhere to department procedures related to waitlisted patients, transferring cases. Contact the CEO when a review is necessary or patient delayed.
