Ensures plan is reflective of admission or outpatient database, on-going findings, age appropriate care, cultural specific needs, and appropriate acuity. Ensures plan is discussed with patient, family/significant others, and completed in a timely manner. Monitors the patients progress based on the plan. Revises plan on ongoing basis based on patient condition and evaluation of progress. Ensures care meets standards of practice. Ensures effective development and completion of discharge plan including discharge barriers and patient/family education. Ensures that patient clearly understands discharge instructions. In outpatient, identify barriers and needs for patient/family education that will facilitate the outpatient medical management plan. Ensures patient safety related but not limited to, medications and procedures utilizing the five rights; patient falls; decubitus prevention and prevention of nosocomial infections. Ensures optimal pain control and patient comfort; identifies and discusses patient anxieties, fears or concerns regarding patient condition, treatment or discharge. Ensures that patient understands medication purpose, side effects, and administration instructions in the hospital as well as at the time of discharge . Documentation: Charting is accurate, legible, dated, and timed. Documentation reflects nursing process and interventions and evaluations taken. Utilizes computer systems effectively and efficiently for optimal patient care. Clinical Outcomes: Discusses patient findings and progress toward outcomes with physicians and other members of the health care team. Demonstrates competencies during the probationary period and ongoing completion by departmental competency validation. Make comprehensive nursing decisions based on interpretation of data, assessments, and evaluations of patient outcomes. Participates in departmental performance improvement activities, i.e., planning, measuring/monitoring, assessing, and improving. Unit Charge - A Unit Charge Nurse is a registered nurse designated by Avera Marshall to have responsibility for care and coordination of care on a particular unit during a particular shift. Meets all of the essential functions of a staff nurse and may have patient assignment. Begins each shift with Daily Line-Up with enthusiasm-stand up, engaging group in questions for discussion, and concludes with prayer. Completes Nurse-Patient Rounds as assigned: focusing on priority questions, immediate follow up with peers on variances identified, providing praise to peers when recognized by patient, and providing completed documentation to nursing supervisor/department leader in a timely fashion. Supports service excellence strategies by ensuring completion of hourly rounding, consistent use of AIDET, and other skills and expectations as defined by the department leader; evidenced by consistent and thorough documentation, and managing up organizational goals and initiatives to peers, patients, visitors, physicians, and community members. Considers factors related to safety, effectiveness, and cost in the planning and delivery of patient care when making assignments, for a specific unit during an assigned shift. Utilizes the Shift Management Tool for documentation of staffing needs and/or variances routinely, and/or utilizes department staffing matrix to staff unit accordingly. Plans for meal breaks at the beginning of each shift, keeps shift report focused on pertinent patient information and on time, ensures OT is minimized, and assures team stays organized to hand off on or before the end of the assigned shift. – Any variances are to be detailed in writing with measures attempted to avoid overtime. Investigates incidents, patient complaints, and care issues and reports to nursing supervisor with suggestions for change/improvement. Reports all AHE and high risk situations to department leader, Assistant CNO, and CNO immediately. Ensure quality care is delivered on all shifts as assigned to Unit Charge role through regular interactions with Unit Charge RNs in each area, and as needed based on activity and needs of department for a given shift.
