The Clinic RN (Registered Nurse) provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants. Assists in the examination, testing, and treatment of participants. Implements orders written by primary care providers. Triages participant and staff phone calls to determine the priority of care and routes accordingly. Schedules outside appointments for participants when necessary. Effectively communicates and actively engages in interdisciplinary team meetings, family meetings, and clinic meetings. Participates within the interdisciplinary team in the formulation of Plans of Care participants, as well as in other interdisciplinary team settings. May be required to provide after hours and holiday coverage. Complies with all regulatory and policy and procedure guidelines. Position Purpose The Registered Nurse delivers high-quality, patient-centered care by applying clinical knowledge and expertise within the context of the nursing process. This includes individualized patient outcomes, coordination of care and educational support to patients. This role works in close collaboration with cardiology physicians and advanced practice providers (APPs) to enhance both patient care and the overall patient experience. Additionally, the RN supports departmental leadership in promoting clinical excellence and facilitating the work of the provider team. Nature and Scope Provides patient care, assessment and planning in close collaboration with the physician, APP and multi-disciplinary teams.
