This role will be Monday - Friday 8AM - 5PM but some longer hours may be needed. After Orientation, there will be an opportunity to work remotely 1-2 days per week. Provides supervision of Licensed Practice Nurse Navigator activities Cultivates and maintains effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care. Communicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs. Facilitates clinical practice guidelines at the patient level. Performs outpatient and clinic care coordination and monitors patients' care amongst the hospital, clinic, and home. Facilitates care via telehealth to assist with patient needs and concerns. Provides health promotion activities and education for caregivers and staff at assigned facility, including workshops and trainings. Refers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel. Coordinates patient placement activities to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff. Communicates and collaborates with leaders and staff at external organizations/agencies regarding placement needs for whole childcare. Maintains assigned databases. Collaborates in collection of data for identification and resolution of flow issues. Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes in system-wide processes in relations with the Behavioral Mental Health strategy. Prepares periodic reports as directed. Participates actively and contributes to organizational initiatives that impact patient flow. Supports continuous performance and quality improvement. Communicates openly with appropriate leadership team to ensure knowledge of key decision factors. Serves as expert clinical resource for patient placement by ensuring smooth functioning of the patient flow process to reduce wait times and avoid unnecessary patient movements. Job Description The Pediatric Infectious Diseases Nurse supports care coordination and clinic visits for pediatric patients with a variety of bacterial, viral, fungal and parasitic infections. This includes challenging conditions like MRSA, osteomyelitis, fevers, bone marrow transplants and organ transplants, HIV, and tuberculosis. S/he works autonomously, utilizing the nursing process to assess, plan, coordinate, implement, and evaluate patient care. S/he uses a high level of clinical knowledge to fulfill patient needs, ensures efficient patient care, and provides patient education when necessary. The Clinic Nurse acts a patient advocate and as a liaison between insurers, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. The Pediatric Infectious Diseases Clinic Nurse is responsible for coordinating outpatient services and needs a panel of pediatric patients and providers in the ambulatory setting. His/her responsibilities include both in-clinic procedures, managing the outpatient infectious diseases clinic flow, as well as assisting with insurance authorization and prior medication authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care.
