Relocation Assistance (based on eligibility) Unit Overview The Ortho/Neuro unit at Duke Regional Hospital is comprised of a 40–bedded floor with Orthopedics and Neuroscience specialties. This is a fast-paced state-of-the-art orthopedic and neurosurgery unit that uses enhanced recovery after surgery principles to both minimize narcotic usage and get the patient home and back to baseline as quickly as we can. The majority of our patients stay 6-24 hours on the unit, in that time they are provided for by a tremendous interdisciplinary team of nurses, physical therapists, occupational therapists, care managers, pharmacists, and doctors — and it’s the teamwork that makes this unit soar.. The nursing unit utilizes Epic for the EMR, Omnicell for the medications, and Pyxis for the patient supplies. Occ Summary The Nursing Assistant (NA) is a patient care support partner with the Registered Nurse (RN) and/or Licensed Practical Nurse (LPN) and performs work as delegated by the RN or LPN. The NA provides direct care to patients and assists with non-direct patient care unit coordination. The individual in this position is expected to perform basic care and comfort standards as well as have knowledge of the patient population needs in the assigned work area. In addition, theNA is expected to assist the RN/LPN in the care of acutely ill patients. This individual is expected to practice medical asepsis, respect the dignity and privacy of every patient and family, and to respect cultural differences during patient care. They are expected to maintain the patient/family environment, perform general unit upkeep, order and maintain unit equipment, and order supplies as needed, with or without direct supervision. The NA is expected to use accurate verbal and written communication and to document patient care appropriately in the electronic health record. Work Performed Prepare patients, equipment and supplies for specific procedures and provide manual assistance as required. Obtain and record patient data for medical records noting and informing RN/LPN of information collected. Administer treatment and personal care procedures to patients including, but not limited to, feeding, bathing, hair care, changing clothing, nail care, bed-making, assisting with ambulation, enemas, skin care, and bowel and bladder elimination; provide such additional care as required to meet the personal needs and comfort of assigned patients. Participate in teaching activities by reinforcing teaching instructed by RN and/or providers as needed. Assist providers and nurses with physical examinations by helping position patients, changing non-sterile dressing and weighing patients. Take and record vital signs, record I&O, and apply ice bags. Note and report any changes in patient's condition. Turn and position patients, set up and feed patients as necessary, provide patients with fresh drinking water. Perform fingersticks for blood glucose testing, with appropriate training, document and notify RN/LPN. Assist with admission, discharge, and transportation of patients. Follow standard precautions and use personal protective equipment as required. Collect, deliver and conduct routine tests on patient specimens. Clean assigned area; stock and replenish supplies and equipment as required. Participate in own professional development by maintaining required skills validation and attending educational offerings. Support the development of other staff and formal learners. May also perform clean dressing change for wounds over 48 hours old. Perform other related duties incidental to work described herein. Knowledge, Skills and Abilities Working knowledge of procedures and techniques involved in administering routine and special treatments to patients. Working knowledge of sanitation, personal hygiene and basic health and safety precautions applicable to work in a hospital. Workingknowledge of the organization and physical layout of the hospital. Working knowledge of infection control procedures and safety precautions. Ability to withstand prolonged standing and walking. This position is very active and requires standing, walking,bending, kneeling, stooping, crouching, crawling and climbing all day. Coordination of hospital admissions from IPOP The Registered Nurse (RN) supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family; assesses the needs of the patient undergoing infusion treatment; plans, implements, and evaluates patient care; and teaches patients, families, orients colleagues and peers. Essential Duties • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. • Use only authorized abbreviations established by this facility when recording information. • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical. • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. • Report all accidents and incidents you observe on the shift that they occur. • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Report any known or suspected unauthorized attempt to access facility’s information system. • Perform only those nursing care procedures that you have been trained to do. • Ensure that the resident’s room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). • Greet residents and escort them to their room. • Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. • Make resident comfortable. • Inventory and mark the resident’s personal possessions as instructed. • Store resident’s clothing. • Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. • Transport residents to new rooms or to the receiving area. • Assist with loading/unloading residents’ to/from vehicles as necessary. • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. • Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Report all complaints and grievances made by the resident. • Participate in and receive the nursing report as instructed. • Follow established policies concerning exposure to blood/body fluids. • Make beds as instructed. • Put extra covers on beds as requested. • Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. • Ensure that residents who are unable to call for help are checked frequently. • Answer resident calls promptly. • Check residents routinely to ensure that their personal care needs are being met. • Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). • Record the resident’s food/fluid intake. Report changes in the resident’s eating habits to the Nurse Supervisor/Charge Nurse. • Keep residents’ water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. • Perform after meal care (i.e., remove trays, clean resident’s hands, face, clothing, etc.). • Participate in and receive the nursing report upon reporting for duty. • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. • Follow work assignments, and/or work schedules in completing and performing your assigned tasks. • Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. • Notify the facility when you will be late or absent from work. • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. • Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). • Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). • Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. • Perform restorative and rehabilitative procedures as instructed. • Assist in preparing the resident for a physical examination. • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). • Report injuries of an unknown source, including skin tears. • Observe disoriented and comatose residents. Record and report data as instructed • Perform special treatments as instructed. • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility. • Use only the equipment you have been trained to use. • Report defective equipment to the Nurse Supervisor/Charge Nurse • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. • Report all allegations of resident abuse and/or misappropriation of resident property. • Honor the resident’s refusal of treatment request. Report such requests to your supervisor. Supervisory Requirements This position has no supervisor responsibilities Qualification Education and/or Experience Must possess, as a minimum, an 10th grade education Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from managers and employees. Mathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Certificates, Licenses, Registrations Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
