Key Responsibilities Assess patients for hospice eligibility and appropriateness of services Provide education to patients, families, and healthcare professionals regarding hospice philosophy, services, and benefits. Collaborate with hospital physicians and Hospice Medical Directors to confirm hospice eligibility. Coordinate and communicate key clinical information to ensure a smooth transition to the hospice admitting nurse. Work closely with hospital case managers, discharge planners, physicians, and other healthcare professionals. Serve as a trusted resource and advocate for patients and families exploring end-of-life care options. Performs resident care when not being utilized as a CRMA. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. High School Diploma or equivalent. 2. Must have a Residential Care Specialist 1, or Personal Support Specialist (PSS) Certificate. May be trained within 4 months of employment. 3. Must have state approved 40-hour certificated medication administration certification. 4. Previous experience in long-term care resident care is preferred. 5.
