A Brief Overview The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with, and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Provides age/developmentally appropriate care. Care is delivered through RN leadership for patient care within the framework of family-centered care. Responsibilities POSITION SUMMARY: The Rehab Liaison receives patient referrals via phone or fax. He/she arranges for evaluations and reviews initial payor data. This individual secures appropriate contact information and assesses patient’s rehabilitation potential and suitability for hospitalization. They interview patient and family to gather information and determine special needs. They must adhere to admission criteria and present potential patients for approval at admissions meeting. They also inform physicians of pending admission and coordinate pertinent data with Business and Admitting Office. They represent the facility in all contracts with other health professionals and the general public in a manner which enhances the facility’s reputation. This position summary contains the most relevant duties and responsibilities and does not exclude other assignments not mentioned. Qualifications QUALIFICATIONS: 1. Two (2) years clinical experience in medical or one (1) year clinical experience in a physical rehab setting preferred. 2. Working knowledge of Medicare guidelines for Rehabilitation. 5. EDUCATION / LICENSURE: 1. If RN, must be Registered Nurse currently licensed in the State of Texas, or part of a compact state license, or has a valid temporary RN license for the State of Texas while waiting to receive the permanent license. BSN preferred. 2. Must have a current RN, Physical Therapy, Occupational Therapy, or Speech Therapy licensure in the State of Texas and maintain licensure and certifications. 3. Must have and maintain current BLCS certification, according to unit specific needs.
