Among the key duties of this position are the following: Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver and DCF/CP&P, as needed. Assesses a child’s heath care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers as needed. Schedules an Initial Physical Health Evaluation, in consultation with the caregiver and DCF/CP&P, reviews the preliminary and final reports, and ensures that all recommendations for health care are completed. Develops a child health care plan in collaboration with the child’s medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF/CP&P. Links the child and caregiver with appropriate health resources and community services. There are two distinct waiting areas, one for pediatric patients and their families, and the other for all other visitors. There are 7 negative flow rooms, one of which is also an isolation room. Clinical and clerical workstations, nutrition areas, and supply and pharmaceutical dispensing machines are centrally located in each zone. A dedicated radiology suite, including 2 CTs, a DECONTAMINATION suite with a separate entrance, as well as a satellite lab are also located within the Emergency Department.
