The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. The Virtual Care Coordinator supports operations of the BOV Telehealth Command Center at Children's National Hospital. Provides virtual surveillance for patients with critical congenital or acquired heart disease in the CICU at Children's National Hospital, as well as others in remote institutions nationally or internationally. Monitors patient's vital signs, trends from multiple monitoring devices, and alerts bedside teams of concerning trends when appropriate. Coordinates nursing care and services for patients and family members in specific healthcare specialty encounters by telephonic, electronic, in person, and/or by mail. Consults with physicians, and other healthcare providers to resolve health concerns. Incorporates all members of the health care team in the planning and delivery of patient care. Deals successfully with complex clinical problems. Conduct quick assessment of patients through the command center and handle emergency situations appropriately. Demonstrate evidence-based practice through participation in unit projects and activities. Responsible to lead the activities of the patient care delivery team for specific patient population. Provide continuous accountability to assess, plan, implement and evaluate clinical care and ensure that continuity of care is maintained. Previous Pediatric Critical Care Nursing Experience Is Preferred. This position is on-site at our main campus (111 Michigan Ave NW Washington, DC 20010). Collaborates with bedside teams to solve clinical patient problems using virtual monitoring and AI tools. Communicates unresolved issues to medical staff or CICU nurse as appropriate. Applies existing basic and advanced features in the Command Center, including AI algorithms in use to enhance assessment and documentation during the virtual surveillance activities. Participates in expanding the command center care model to other inpatient, ambulatory, and emergency room care models and enhancing AI tools. Job Summary and Responsibilities The job is responsible for providing consultative patient care nursing services via the centralized Virginia Mason Franciscan Health (VMFH) Virtual Health Network which focuses on the provision of virtual remote care for patients in intensive care units (ICU), other inpatient care units and/or other CommonSpirit Health markets. From a centralized command center location, an incumbent works collaboratively with the Tele-ICU and an interdisciplinary team of healthcare providers to facilitate achievement of clinical objectives, quality/customer service standards and improvement of patient care outcomes. The provision of nursing services are extended to CommonSpirit markets in other states outside Washington State and will require procurement of current RN licensure in those states. Work is conducted in accordance with VMFH-FH policies, applicable regulatory requirements and scope of practice for the nursing profession and is strongly focused on patient safety. Work involves continuous contact with physicians, patients, patient families, community agencies, patient care staff and management. An incumbent is accountable for the quality of care provided to patients, and has the authority to direct care, provide education, seek resources at the unit/department level, and delegate appropriate tasks. Work also includes: 1) collaborating with patients, families and on-site healthcare providers in developing/implementing/assessing the patient's plan of care and appropriate nursing interventions for optimal patient care/safety; 2) monitoring electronic systems/equipment to identify variations from standard norms relating to patient care/status; and 3) providing education/guidance on best practice intensive care nursing and ensuring that providers are following established patient care procedures and documentation standards. Essential Function Monitors assigned patients from a command center remote location via live streaming video equipment; makes ongoing assessments with respect to physical, psychosocial, cultural, behavioral/emotional, developmental and spiritual needs; communicates with members of the Tele-ICU Center team across all locations and the bedside ICU or other inpatient care interdisciplinary team(s) in a collegial manner to provide safe and error-free care during routine and emergent conditions. About the Role As a Devoted Preventative Primary Care Provider, you’ll be part of a mission-driven, interdisciplinary clinical team focused on improving the health, dignity, and quality of life of our members.
