Job Description: The Travel Clinical Team Manager is a registered nurse who oversees the implementation, supervision and oversight of all patient care programs, achievement of both clinical and administrative outcomes, and professional and supportive services to clients in a safe, cost-effective manner at a designated service line branch. The Travel CTM is responsible for the coordination of care during the plan of care benefit period, clinical practice, staff engagement, safety and the wellbeing of staff and patient care, for compliance of corporate standards as they apply to the overall mission of the organization under a temporary assignment. The Travel CTM will receive temporary assignment from the Area Vice President or designee based on branch staffing needs. JOB REQUIREMENTS: Graduate of an accredited Diploma, Associate; or a bachelor’s degree from a School of Nursing. Minimum of 3 years’ relevant experience as a registered nurse in a Home Health service or Hospice environment along with being in a leadership role. Current State License as a Registered Nurse in all states assigned in travel region. KEY ACCOUNTABILITIES: Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice and Skilled Home Health team. Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care. Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Plans, directs, and participates in delivery of professional services for patients. Participates in the organization’s strategic planning activities. Focuses on services toward diagnosis-specific client care needs. Assists in the development, implementation, and evaluation of policies, procedures, clinical protocols, forms, quality improvement monitors, service programs, and other components of program infrastructure annually. Implements and performs liaison activities with both individual clients and the community. Maintains staffing levels at branch locations to meet patient needs. Supervises and provides direction to agency personnel, and the coordination of services with providers, vendors, or facilities in the delivery of care and related issues. Supervises and provides clinical direction to the Registered Nurse, Licensed Practical Nurse, Spiritual Care MSW, Music Therapy, Massage Therapy, Volunteer, and Bereavement teams to ensure quality and continuity of services provided and oversees the office functions at assigned travel branch. Assures compliance with the requirements of state licensure, Medicare certification, and any other applicable oversight agencies through policy and procedure development, staff education, and ongoing monitoring activities. Coordinates services to ensure the availability and competency of staff to respond to agency referrals and to provide assessment visits within 24 hours of referral. Processes and coordinates patient referrals, physician orders, staff assignments, patient re-certifications, and communications between various caregivers, the patient and their family, and the referring physician. Meets with all field staff on a regular basis to provide guidance and information related to specific issues. Oversees the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certifications standards and legal and ethical imperatives. Oversees and provides leadership to Clinical Supervisors, Office, and Field Staff, to ensure the delivery of safe, effective, and appropriate home health services and discusses and directs the field staff assignments for patient care. Identifies and instructs staff on admission criteria for new clients and evaluates eligibility for home health and hospice benefits. Promotes compliance with all fiscal intermediary and/or other third-party payers, through education, and coaching. Stays informed of current Medicare, Medicaid, and third-party reimbursement regulations and changes and ensures compliance. TRAVEL: 100% Travel Meets all applicable agency policies and procedures related to health screening and required testing.
