Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs. Qualifications Must meet one of the following requirements: Have a minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility) Have 1 year of experience within the last 5 years in direct client care in a home health or hospice setting Have satisfactorily completed a training and competency evaluation program that complies with federal and state regulations Be registered and listed in good standing on the Nurse Aide Registry in the state of employment Must possess a valid state driver’s license and autombile insurance and have reliable transportation. JOB SUMMARY The Nurse supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Nurse will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility. SUPPORT DUTIES Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of Alzheimer’s dementia and other diseases that cause dementia. Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff. Assist in oversight of the Memory Care Aides and Certified Nursing Assistants (CNAs), including supervision and assistance with ADL care, transportation, and non-scheduled activities by staff. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail. Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training. Encourage family involvement through volunteering and visitation. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C.
