Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred. DELEGATION OF AUTHORITY: Authority is delegated to the individual in this position to: 1) provide care to residents following facility policies and procedures 2) report residents' needs and concerns to a licensed nurse DUTIES AND RESPONSIBILITIES: 1. Follow written and oral directions 2. Follow established performance standards and perform duties according to nursing service policies and procedures. 3. Assist new employees in following established facility policies and procedures, 4. Identify resident problems and concerns and report them immediately to a charge nurse or a licensed nurse. 5. Complete assignments timely, completely and accurately. 6. Conduct resident rounds as assigned. 7. Initiate corrective action as necessary and/or seek assistance of a licensed nurse. 8. Identify safety hazards and emergency situations, and initiate corrective action. 9. Attend all in-service classes as assigned and complete assignments. 10. Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit. 11. Document incidents and/or unusual problems according to established facility procedures. 12. Participate in interdisciplinary team and other facility meetings as assigned. 13. Participate in the development of an individualized plan of care of assigned resident. 14. Review care plans and perform nursing care as outlined. 15. Document in the nursing assistant notes the care and treatment provided to the resident and the resident's response or lacke of response to care provided. 16. Report changes in residents' condition to Charge Nurse or Nurse Supervisor. 17. Follow facility procedures for admission, discharge and transfer of residents. 18. Complete documentation accurately, timely and following facility policies and procedures. 19. Listen to resident and family complaints and report problems to Charge Nurse. 20. Assure residents have call lights at hand and answer call lights promptly. 21. Assure residents are given adequate assistance with meals, nourishment and transportation to meal service area. 22. Consistently work coopertively with residents, charge nurses, treatment nurses, restorative nurse, other nursing assistants, physicians, families, consultant personnel and ancillary service providers. 23. Observe all facility safety policies and procedures. 24. Come to work in a clean, neat uniform and consistenly present an appropriate professional appearance. 25. Come to work as scheduled and consistently demonstrate dependability and punctuality. 26. Assume accountability for data contained in the employee's handbook. 27. Perform other related duties as directed by his/her supervisor. 28. Perform all duties assigned in an effective, timely and professional manner. 29. Observe infection control procedures. 30. Follow Residents' Rights policies at all times. 31. Be responsible for the safety of residents under his/her supervision. 32. Observe all facility policies and procedures. 33.
