Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner. Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required. Prepares and administers medications and performs treatments as ordered by the physician. Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Counts all controlled drugs with other nurse at shift change and notifies management of all drug and narcotic discrepancies noted on your shift. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Makes written and oral reports/recommendations concerning the activities of shift as required. Admits, transfers and/or discharges residents as necessary to meet the resident’s individual needs and maintains the overall safety of other residents, staff and visitors. All other duties, as assigned. Registered Nurse (RN) Skills and Abilities: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Ability to organize and prioritize work. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Registered Nurse (RN) Education and Experience: Current CPR Certification required. Nursing Degree or diploma from an accredited College or University. Possess a current license to practice in the State as a Registered Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required. Staff on this unit are highly proficient in pre-operative and intra-operative support as well as immediate post-operative phases of cardiovascular procedures. Acute cardiology and heart failure patients also receive care in CVICU, along with overflow medical and surgical intensive care patients. Staff expertly manage all types of invasive lines, medications, hemodynamic needs, intra-aortic balloon pumps, continuous dialysis and ultrafiltration machines, and various ventricular assist devices. The success of their nursing staff is their number one priority and is supported through their encouraging environment of comradery and mentorship. The MA-Float may be asked to provide coverage outside of their primary center. Duties/Responsibilities: Conduct intake with patients, measuring and recording vital signs, patient interview, and medical history; perform examinations and routine screenings, ensuring patients are informed and all questions and concerns are addressed for each procedure. Critically listen and engage with patients throughout appointment, ensuring needs and/or changing in patient conditions are reported to PCP, specialists, and/or other clinical personnel. Perform all tasks with a hyper focus on patient experience, including processing of patient phone messages, including returning calls; routing calls to other team members as appropriate; calling patients to obtain and relay pertinent information for the physician. Change dressings, apply bandages, remove sutures and other first aid procedures; use CPR skills, when necessary, all under physician supervision. Ensure compliance with OSHA safety standards and conduct accurate, legal, and ethical documentation at all times. Provide health coaching to a defined group of patients to support healthy lifestyle choices; follows up with coached patients via weekly calls. A minimum of 1 year work experience as a Medical Assistant. 6+months' phlebotomy experience is highly desired. BLS for Healthcare Providers required. EClinical Works or similar EMR experience. Duties & Responsibilities Per the hospital Utilization Management Plan, the RN CM performs utilization and resource management by assessing and reassessing the patients’ medical necessity to ensure accurate status (i.e. inpatient, observation, or ambulatory care) and documents the timely, appropriate use of evidence-based criteria, e.g. InterQual. Responds to all insurer requests for information and collaborates with onsite review nurses e.g. regarding insurance authorization. Conducts medical record review for level of care (LOC)/disposition planning. Reassesses LOC timely to ensure appropriateness. Discusses LOC and medical necessity with attending physician daily. Communicates to patient and/or patient’s families, e.g. medical necessity, goal length of stay, projected discharge date. Identifies, actively intervenes escalation of system delays for secondary review, and provider-related delays for secondary medical review (i.e. physician advisor, or Chief Medical Officer) and tracks delays e.g. potential or actual avoidable days. Proactively identifies patients readiness to transition and intervenes timely, e.g. downgrade, or discharge to a post-acute care. Screen surgical procedures for medical necessity, e.g. SIMS plus. Identifies potential barriers patients may experience with transition, and works actively to prevent barriers and facilitate timely care and transition planning. Discusses care requirements, and assures documentation is provided for the post discharge/transition care setting, e.g. concerning patient’s risk for readmission and pertinent discharge/transition information. Initiates timely referrals to post-acute care providers, e.g. home health agencies, skilled nursing facilities, long-term care hospitals, acute rehabilitation facilities, assisted living facilities, homeless shelters, etc. Coordinates patient care conferences, e.g. patients readmitted within thirty days of discharge, or patients who have exceeded thirty days of stay. Identifies and recommends opportunities for improving quality and customer service, and actively participates in performance improvement activities. Participates in peer review/audit, and periodic interrater reliability evaluations, e.g. competency assessments and subsequent coaching. Develops a plan for attaining professional development goals e.g. set during performance review. Identifies patients with complex care needs. Collaborates in the development, notification and maintenance of complex care plans. Actively participates in problem-solving activities. Actively participates in internal and external committees, and projects especially those that address quality, patient flow, LOS and revenue e.g. UR Committee, Long LOS, ICD 10, Patient Financial Services, Crimson and Patient Throughput Team.
