Communicate care plans, testing needs, and clinical updates across inpatient and outpatient settings and ensure seamless transitions throughout the transplant and donation processes. Present cases, evaluation findings, and updates during weekly multidisciplinary meetings and selection committees. Collaborate with the HLA laboratory on crossmatching, donor preselects, match reviews, and NKR-related logistics. Facilitate clear communication regarding candidacy, closure of referrals, and evaluation outcomes with transparency and consistency. Promote continuity, safety, and consistency of care across all phases of transplantation and living donation. Education & Counseling Provide individualized and group education regarding the transplant process, testing, surgery, medications, and long-term expectations. Facilitate informed consent discussions and reinforce patient and caregiver understanding. Support education on medication adherence, infection prevention, and self-management strategies. Assist in developing educational materials and participate in community or outreach education efforts as assigned. Regulatory & Data Management Maintain accurate and timely documentation in the EMR and transplant databases (UNOS, Phoenix, SpectraPath, NKR, etc.). Ensure compliance with institutional, CMS, and UNOS requirements for data accuracy,timeliness, and completeness. Participate in audits, data validation, and chart reviews to maintain readiness for regulatory inspections. Collaborate with quality and data teams to ensure appropriate reporting of clinical outcomes and key performance indicators. Quality, Safety & Performance Improvement Identify opportunities for process improvement and contribute to quality initiatives within the transplant program. Report and track patient safety events, readmissions, and adverse outcomes. Support root cause analysis, process corrective action plans, and QAPI documentation. Promote adherence to program protocols and assist in development and revision of clinical policies and pathways. Professional Development & Program Support Maintain current transplant and donor specific knowledge through continuing education and professional development. Attend required meetings, including selection committee, morbidity & mortality, and program quality reviews. Support program coverage needs across coordinator roles as necessary. Attention to detail is critical, as is being observant and following directions. REQUIREMENTS: Registered Nurse with current license in the state of employment. 2 years of hospice experience preferred. Previous experience with clinical management, hospice case management, or other relevant clinical experience. Valid driver's license and auto insurance in your name as a driver. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
