Qualifications The ideal candidate for the Nurse Navigator (RN) will have the following background and experience: Bachelor’s prepared Registered Nurse (BSN). Oncology Certification (OCN) preferred. Nurse Navigator Certification a plus. 5 (five) years work experience as a Registered Nurse in an oncology practice or clinic. Demonstrated leadership, interpersonal and collaboration skills. Ability to apply the full nursing process in day-to-day clinical operations, including assessment, analysis and nursing diagnosis, care planning, implementation and outcomes evaluation. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. Overview The experienced staff RN is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame. Responsibilities Provides care to these patients via therapeutic use of self, the nursing process, the therapeutic environments, treatment and modalities and interdisciplinary collaboration. Perform initial physiological and psychosocial needs assessment on all patients within 30 minutes of admission and shift assessment within 2 (two) hours if coming on duty. The RN shall, for their patients, collaborate in the formulation of the overall plan of care for the patient, as evidenced by written documentation.
