Any concerns noted, patient status; is legible, neat; is accurate, thorough, completed in a timely manner. Documents relevant to the Medication Administration, Narcotic Administration, and accountability and destruction of medication following patient death. Care Plans are kept current, applicable to patient and patient needs. Incident Reports are completed accurately and submitted immediately to the Nursing Support Service Manager/designee. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C.
