The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. Other duties as assigned. REQUIRED Active and valid state-issued Certified Nursing Assistant (CNA) license Current CPR certification or the ability to obtain CPR certification. Nursing Assistants are responsible for working closely with the nurse to provide excellent direct patient care. This person supports activities of daily living (ADL's) with patients and prevents complications. Roles and Responsibilities: Direct Patient Care- Toileting, ambulating, repositioning, monitoring and recording intake and output of fluids, recording patient weight, obtaining vital signs, and reporting abnormal findings or changes in the patient's conditions to the RN. Reinforcing education with patients; i.e. incentive spirometer use, and fall prevention. Maintaining a clean and organized patient bedside area to ensure a safe workspace. Intentionally rounding on each patient every hour. Mandatory Orientation Training completion required: one full week of Monday through Friday 8a-4p prior to starting work on the unit. Required Skills and Experience: High school or equivalent experience BLS- will be provided if needed Preferred Skills and Experience: NYS Nursing Assistant Certification (CNA) Previous patient care experience Physical Requirements: Frequent hand and finger use (e.g., to operate a computer keyboard). Lifting and carrying items up to 40 lbs. (e.g., boxes, equipment, IV solutions), with or without reasonable accommodation. Ability to stoop, kneel, or crouch for tasks such as retrieving items from lower shelves or filing in low drawers. Ability to sit, stand, or walk for extended periods, as the position may require. Capacity to operate job-related equipment, with or without reasonable accommodation.
