MDS COORDINATOR- Exempt (Salary) Position Reports to Administrator OVERVIEW: MDS Coordinator administers patient assessments and overseas the assessment process, setting the assessment schedules and assuring that assessments are done in an accurate and timely manner. QUALIFICATIONS: Proficiency in MDS 3.0. Demonstrating knowledge of state and federal regulations. Registered Nurse or LPN with current, active license in the state. Minimum two years of clinical experience in LTC setting. PHYSICAL REQUIREMENTS: This position is very active and requires standing for long periods of time, walking throughout facility, bending, kneeling, stooping all day. The specialty Nurse Practitioner (NP) or Physician Assistant (PA) in the ambulatory care setting works in collaboration with a multidisciplinary health care team. The specialty NP or PA is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The care is focused for patients with pancreatic disease, including acute and chronic pancreatitis, pancreatic cysts, and exocrine pancreatic insufficiency. The NP or PA functioning primarily in a clinic setting with no weekend or after hours call schedule and no inpatient responsibilities.
