Facilitates safe patient transitions to the next care setting as part of plan of care. Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care. Focuses on quality outcomes of patient care when addressing customer’s needs to include current evidence based practice and quality measures. Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Educates by assuming responsibility to individualize and coordinate patient education, and discharge specific education that includes family/caregiver. Evaluates for patient comprehension and documents. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Ability to delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Able to recognize and respond to emergency situations. Performs routine procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury as privileged by the Genesis Credentialing Board. 5. May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and Genesis Credentialing Board. 6. May need to take call and work some off-hour shifts as needed. 7. Perform in-hospital consultations.
