ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. Conducts urgent physical and psychosocial assessments of the patient and family during after hours and determines appropriate interventions. Provides and maintains a safe environment for the patient. Provides professional nursing care by utilizing all elements of the nursing process. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and family regarding diagnosis, medications, treatments and progress. Documents patient care thoroughly and timely. Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. Frequently observed ailments include COPD exacerbation, respiratory viruses, asthma, bronchitis, etc. The IMC is a brand new 21-bed critical care step down unit that provides enhanced nursing services for patients that are too sick to be safely provided care on a medical/surgical unit but may not be in need of the monitoring available in the ICU. Our patients in the IMC are mixed acuity, which means you may encounter cardiovascular, neurological, respiratory, urological, gastrointestinal, or endocrine step down patients. How does scheduling work? Scheduling is observed in a four-week period. The nurse manager inputs the schedule and pays close attention to staff requests including desired schedule patterns and vacation times. Job Objectives: Assumes direct responsibility and accountability for, and authority of, the nursing care of all patients assigned. Responsibilities Assesses and identifies the patient's physiological and psycho-social problems and needs. Develops an individualized plan of nursing care for each patient in which goals, in terms of measurable outcomes, are stated. Ensures that nursing care plan is consistent with the medical plan and adheres to accepted current standards of care and practice. Makes appropriate assignments to team members by utilizing patient acuity data, and maintains accountability for members under his/her direction. Ensures nursing care adheres to current hospital/departmental policies, procedures, and regulations. Demonstrates principles of infection control and collaborates with infection control nurse to ensure safety of patients, staff, and visitors. Participates in the orientation of new personnel.
