Job Description Summary: An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home, or in an alternate infusion site, to ensure patient safety with continuity and compliance under a physician’s plan of care. Location: Acute Care Hospital Tower Shift- 12hr Days (7am-7pm) Work Hours: This program is a 7 day a week service, rotating every other weekend and rotating holidays Employment Type: Part Time with benefits *must be able to work a minimum of 24 hours per week* **This position will be onsite and candidates must have the ability to travel within a 20 mile radius approximately 50% of the time Hospital at Home is an innovative program that combines in-home with remote monitoring, offering patients timely, hospital-level care in their homes. Though the list of diagnoses enrolled in Hospital at Home continues to expand, common disease states include acute pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, or exacerbations of chronic conditions, such as heart failure or chronic obstructive pulmonary disease. For many patients, the program provides an alternative to the traditional hospital setting and results in improved patient outcomes and satisfaction. The Hospital at Home team is looking for motivated nurses with critical thinking skills, acute care experience, and an interest in serving the Parkland community. Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate acute patient care delivery in the patient home. Using the nursing process, the RN assesses the patient in their environment, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response.
