Coordinates patient care; gives explanations and teaching to patient/family, as necessary. Obtains, documents and assesses vital signs per protocol. Ensures timely submission of the MDSs to the State with proper follow-up on validation errors. Maintains validation records from the submission process in a systematic and orderly fashion. Qualifications Graduate of an approved Registered Nurse / License Vocational Nurse program and licensed in the state of practice required. Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case- Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. Location: Moody Outpatient Center This role functions as an integral member of the Moody Breast Center, on the campus of Parkland Hospital, in providing direct patient care. This role assists physicians, mid level providers, nurses, and other medical staff by performing administrative and clinical duties under the direction of physician. This role reports to the Director, Radiologic Imaging, or designee, who is responsible for administrative supervision and orientation to the area's policies and procedures. This role obtains vital signs, perform preventive screenings, maintains an inventory of appropriate supplies, ensures safe and efficient patient flow, tracks high-risk referrals and lab tests, facilitates safe transitions of care, organizes the practice environment to assure patient safety and quality health outcomes, and contributes to HEDIS and designated quality measures. The individual displays responsible behaviors, communicates effectively to others and functions at the highest level based on scope of practice. Hours for this position are Monday through Friday, day shift. The Moody Breast Center is closed nights and on holidays.
