Brief summary of purpose: The Assessment Nurse Case Manager completes face-to-face home visits for new enrollees within 30 days of enrollment to onboard and completes regulatory assessments. Scheduled Weekly Hours: 32 - Performing under the direction of the RN/LPN and as a member of the unit-based patient care team, the Certified Nursing Assistant performs duties of direct and indirect patient care.
